Objective:In this study,we observed the clinical effect of modified Yanhusuo Decoction on primary dysmenorrhea of Qi stagnation and blood stasis type and its influence on RI and PI of uterine artery blood flow on the first day of menstruation.We also discussed the results,providing more treatment options for primary dysmenorrhea of traditional Chinese medicine,and providing more objective basis for the clinical application of new ancient prescriptions.Method:In this study,a randomized controlled trial was conducted.The subjects were screened strictly according to the diagnostic criteria,inclusion criteria and exclusion criteria.Sixty patients who met the criteria were selected and randomized into two groups.Sixty patients were randomly divided into treatment group and control group.They were given Yanhusuo Decoction Modified and Analgesic Tablets respectively for intervention.Two groups of patients began to take medicine one week before menstruation,one dose a day,decocted in water,three times,until the end of menstruation,three consecutive menstrual cycles for a course of treatment.The changes of visual analogue VAS score,pain score,TCM symptom score,PI and RI of uterine artery blood flow on the first day before and after treatment were counted,and the comparative data were analyzed to make an objective and fair evaluation.Result:(1)Before the beginning of the study,the general data and indicators of the two groups were statistically analyzed.There was no significant difference between the two groups(P>0.05).The baseline of the general data was the same and it was clinically comparable.(2)Total effective rate:the total effective rate of treatment group was83.33%,and that of control group was 60%.After three weeks of intervention,there was significant difference between the two groups(P<0.05),while the clinical effect of treatment group was better than that of control group.(3)VAS visual analogue score:After clinical intervention,the VAS visual analogue score of the two groups were lower than before,and the difference was statistically significant(P<0.01);after treatment,the VAS scores of the two groups were compared,and there was no significant difference between the two groups(P>0.05).(4)Score of dysmenorrhea symptoms:After treatment,the score of dysmenorrhea symptoms in the two groups was lower than that before treatment,and the difference was statistically significant(P<0.01);after treatment,the score of dysmenorrhea symptoms in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).(5)TCM syndromes integral:After treatment,the total integral of TCM syndromes in both groups decreased,and the difference was statistically significant(P<0.01);after treatment,the TCM syndromes integral of the two groups were statistically analyzed,and the difference between the treatment group and the control group was statistically significant(P<0.05).(6)Single integral of TCM syndromes:After treatment,there were significant differences in abdominal pain,menstrual volume,menstrual color,nausea and vomiting between the two groups and the control group(P<0.05);there were significant differences in breast swelling and pain between the control group and the treatment group(P<0.05);the duration of pain,chest tightness,lower abdominal distention and pain refusal to press,lumbosacral distention and anal distention between the two groups were significant differences(P<0.05).The curative effect was similar(P>0.05).(7)Uterine artery blood flow:PI and RI in the treatment group decreased after treatment,with statistical significance compared with before treatment(P<0.01);PI and RI in the control group decreased after treatment,with statistical significance compared with before treatment(P<0.05);PI and RI in the treatment group were compared after treatment,with statistical significance compared with the control group(P<0.05).(8)Follow-up comparison between the two groups:After three months of follow-up,the recurrence rate in the treatment group was lower than that in the control group,which proved that the treatment group was superior to the control group in the treatment of primary dysmenorrhea of qi stagnation and blood stasis type.Conclusion:Yanhusuo Decoction can treat primary dysmenorrhea of Qi stagnation and blood stasis type by promoting Qi and activating blood circulation,regulating menstruation and relieving pain.The synergistic effect of various Chinese medicines may affect uterine artery perfusion,improve the special state of uterine artery high resistance and low flow in dysmenorrhea patients,reduce blood flow resistance and increase local blood supply,so as to achieve the therapeutic purpose.Modified Yanhusuo Decoction can reduce the degree of pain,alleviate the accompanying symptoms of dysmenorrhea and improve the quality of life of women.For patients with primary dysmenorrhea of Qi stagnation and blood stasis type,it has good clinical efficacy,no side effects,and is not easy to recur,which is worthy of clinical promotion.
