Study On The Mechanism Of The Effect Of Needling At Guanyuan Point On The Hemodynamics Of Uterine Arteries In Patients With Primary Dysmenorrhea

ObjectiveIn this study,acupuncture was used as the study method,the Guanyuan acupoint(RN4)was selected as the stimulation point,and Color Ultrasonic Doppler technology was used to observe the changes in uterine blood perfusion before and after needling at RN4 in PD patients.At the same time,we observed the changes of pain before and after acupuncture of PD patients.We hoped to explore part of the uterine artery hemodynamic mechanism of needling at RN4 in the treatment of PD from the perspective of uterine blood perfusion.MethodsA total of 36 PD subjects were included in this study,and they were randomly divided into two groups: group A(Guanyuan acupoint group)and group B(Not meridian or acupoint group),with 18 cases in each group.The subjects in group A were treated with acupuncturing at RN4,and the course of treatment was 3 menstrual cycles.While the subjects in group B received needling at non-point of 3 menstrual cycles.In addition,18 healthy subjects were included as group C(Healthy group)and did not receive any treatment.Record the pain visual analogue scale(VAS)scores and the peak systolic velocity/end diastolic flow velocity(S/D),pulsatility index(PI)and resistance index(RI)values of uterine artery blood flow of the menstrual cramps within 48 hours before and after the test in group A and B.For group C.We only measured the S/D、PI、RI value within48 hours of their menstruation.We used software SPSS25.0 to statistically analyze all test data and draw conclusions.Results1.There is no statistical difference in comparison of baseline demographic data(age,anteroposterior diameter of uterus)of group A,B,and C,and disease data(disease course,VAS score,S/D value,PI value,RI value)of group A and B(P>0.05),which means that the groups were comparable.2.Before the test,compared with the Healthy group(group C),the Guanyuan acupoint group(group A)and the Not meridian or acupoint group(group B)had higher S/D,PI,and RI values,and there was a statistical difference(P<0.05).3.After the test,the VAS score of group A was significantly lower than that before the test,and the changes had a statistical difference(P<0.05).However,the change in group B was not obvious(P>0.05).Comparing the gap value between the VAS scores of the two groups A and B before and after the study,the results showed statistical differences(P<0.05).4.After the study,the S/D,PI,and RI values of group A decreased compared with those before the test,and the changes were statistically different(P<0.05).However,the change in group B was not obvious(P>0.05).And Comparing the gap value between the S/D,PI,and RI values of the group A and B before and after the study,the results showed that there was a statistical difference(P<0.05).Conclusions1.Needling at Guanyuan point can relieve the pain degree of PD patients.2.High-resistance and low-velocity are typical feature of uterine artery hemodynamics in PD patients.3.The mechanism of needling at Guanyuan acupoint in the treatment of PD may be related to the following points: changing the characteristics of high-resistance and low-velocity blood flow of uterine artery in PD patients,improving uterine blood perfusion,inhibiting excessive uterine contraction andrelieving or even completely dismissing the spasm of the myometrium.
