| Based on the theory of transfusion combined with vibration,clinical trials were carried out to investigate the mechanism of acupuncture combined with vibration system on brain by observing the clinical efficacy of acupuncture combined with vibration system on children with cerebral palsy.Through clinical research,the clinical trial of acupuncture combined with vibration in the treatment of infantile cerebral palsy provides the basis for clinical treatment of infantile cerebral palsy.The general situation of brain development of infantile cerebral palsy was evaluated by head MRI imaging combined with ultrasound,and the curative effect of development and motor behavior was quantitatively evaluated by Alberta scale and Gesell scale.Ultrasound was used to detect the small size of infantile cerebral palsy.The cross-sectional area of muscles in children with cerebral palsy was evaluated by the cross-sectional area of medial head of gastrocnemius muscle of bilateral lower limbs in three groups.AIM: To evaluate the clinical manifestation behavior of children with cerebral palsy before and after treatment by using Alberta scale and Gesell scale;to evaluate the brain development of the three groups before and after treatment by using head magnetic resonance imaging combined with muscle ultrasound;to measure the cross-sectional area of medial head of gastrocnemius muscle of lower extremities of children with cerebral palsy by using muscle ultrasound,so as to evaluate the muscle development of children with cerebral palsy.To explore the improvement of nerve,muscle and brain development in children with cerebral palsy by systematic treatment of acupuncture combined with vibration.Materials and Methods: From March 2016 to May 2018,90 full-term infants with CP encephalopathy were admitted to the Rehabilitation Department of Shenyang Children's Hospital.Among them,there are 50 boys and 40 girls.According to the digital table method,the children with CP were randomly divided into three groups:acupuncture group(30 cases),vibration group(30 cases)and vibration group(30cases).Neurobehaviology and motor behaviour of children with cerebral palsy were evaluated by Alberta motor scale and Gesell whole body development scale before and after treatment.Brain development was assessed in three groups according to the etiology and pathogenesis of cerebral palsy by MRI and ultrasonography.The cross-sectional area of medial head of gastrocnemius muscle of lower limbs was measured by muscle ultrasonography.Result:1.Albert exercise scale was used for comparison before and after treatment.There was no significant difference between the three groups in using Albert Motion Scale before treatment(P > 0.05).After two courses of treatment,the Albert Motion Scale of the three groups was significantly improved(P < 0.05).Among the three groups,the increase of acupuncture combined with vibration group was the largest,significantly higher and better than that of the other two groups(P < 0.05).There was no significant increase in the scores of the motor group,but P < 0.05 also had statistical difference.2 Comparison of Gesell Development Scale before and after treatment There was no significant difference between the three groups before treatment with Gesell Development Scale(P > 0.05).After two courses of treatment,the Gesell Development Scale(Gesell Development Scale)was significantly improved(P <0.05).However,the data of the acupuncture plus vibration group was the highest,and the data of the acupuncture group was the second,compared with that of the pre-treatment group(P < 0.05).After 2 courses of treatment,the Gesell Development Scale was significantly improved(P < 0.05).After the treatment of one course,compared with the acupuncture plus vibration group,the Gesell score of the vibration group did not increase significantly(P < 0.05),with statistical significance;compared with the acupuncture group,the value of the vibration component also did not increase significantly(P < 0.05),with statistical significance.3 Compare the results of maximal cross-sectional area of gastrocnemius before and after treatmentAfter treatment,the cross-sectional area of the medial head of gastrocnemius muscle of the left and right lower limbs in the acupuncture plus vibration group was better than that in the other two groups(P < 0.05),with statistical significance;compared with the vibration group,the cross-sectional area of the medial head of gastrocnemius muscle of the left and right lower limbs in the acupuncture plus vibration group was significantly lower than that in the acupuncture plus vibration group(P < 0.05),with statistical significance.Comparisons of the results of head MRI before and after treatment Cerebral edema often occurs in children with CP due to cerebral hypoxia and insufficiency of blood supply.It is often shown by MRI imaging.Magnetic resonance T1W2 imaging showed diffuse hyperintense signals in the brain parenchyma,while brain edema was often accompanied by subcortical white matter,stenosis of the medial ventricle,and spotted and flexed hyperintense imaging of the white matter.If the signal of posterior branch of internal capsule is abnormal,there will be subarachnoid hemorrhage,hemorrhage under the medial ventricle,cerebral haemorrhage,and even symptoms of cerebral infarction in severe cases.The clinical manifestation of cerebral hemorrhage is high signal imaging in the blood supply area,and low signal imaging is cerebral infarction.Some children with severe CP may also have pathological changes of basal ganglia and thalamus,and uneven high-signal imaging of T1W1 with spot-sheet symmetry,which is caused by lesions of lenticular nucleus,globus pallidus and lateral thalamus.After two courses of treatment,the sulcus,ventricle and subarachnoid space of the three groups were significantly enlarged and widened,and the range of peripheral white matter was gradually narrowed.Softening lesions were found in the lateral paraventricular white matter:high signal intensity patches were seen on T2 WI,and incomplete margins of the ventricles appeared.The extent of enlargement and broadening of sulcus and ventricle in the acupuncture group was significantly less than that in the other two groups.Paraventricular white matter softening gradually decreased and white matter gradually increased.However,no vacuole or multifocal leukomalacia has been observed.Theimaging of myelination degree in vibration needling group and acupuncture group was also better than that in vibration group.5 Contrast of lateral ventricle body ultrasonography before and after treatment After 2 courses of treatment in each group,the results were lower than those before treatment,and the results of vibration needle group were significantly lower.The results of acupuncture group and vibration group were not obvious,and there was a statistical difference between the two groups(P < 0.05).If the three groups were compared before and after treatment,the results of the vibration needle group and the acupuncture group were significantly lower than those of the control group(P < 0.05).Conclusion:1.Cerebral palsy in children is accompanied by obvious nerve and muscle system development retardation.Vibration combined with acupoint allocation therapy can significantly improve the data score of Alberta and Gesell development scale in children with CP.3.Imaging and MRI evaluation of children with cerebral palsy showed that the vibration needle group had better effect than the acupuncture group on improving the brain injury of children with cerebral palsy.Compared with the two groups,the enlargement of lateral ventricle and sulcus and edema were observed.The range of paraventricular leukomalacia decreased and that of white matter increased.No cavity and large area of multifocal leukomalacia were observed.4.Vibration combined with acupoint allocation of transfusion and simple acupoint allocation of transfusion have obvious advantages in reducing the expansion of contralateral ventricles.5.The cross-sectional area of medial head of gastrocnemius muscle of bilateral lower extremities was higher than that of simple acupoint-matching therapy. |
