| [Background]Chronic Obstructive Pulmonary Disease(COPD)is one of the most common respiratory diseases;its morbidity and mortality increases year by year throughout the world.Long disease duration of COPD and its complications that occur during the progression of the disease leads to organ damage,eventually resulting in patient death.After years of clinical studies and practices,the control of COPD symptoms with modern medicine is much more mature,but there is still no effective means to prevent the occurrence of the disease or to prevent the disease from further exacerbation.COPD belongs to the category of ’Lung Distention’ in Traditional Chinese Medicine,and there are unique prevention and treatment ideas in the TCM theory.Oral herbal paste medication is a unique treatment method of Traditional Chinese Medicine.However,not many researches on the therapeutic effect of oral herbal paste for the treatment of COPD have been conducted for the time being.In this study,oral herbal paste medication was selected for the study of ’Lung Distention’ patients with the’Deficiency of Lung and Spleen’ syndrome.This may reflect the variety of treatment methods in Traditional Chinese Medicine and provide more ideas for the treatment of this disease.[Objective]To evaluate the clinical efficacy of TCM on the treatment of stable COPD under the guidance of ’Preventive Treatment of Diseases’ theory,and to verify the effectiveness of the intervention treatment of ’Shu Yu Wan’ herbal paste on stable COPD patients,including improvments in lung function,times of common cold and/or acute exacerbations during the observation period and clinical symptoms.[Methods]60 Chronic Obstructive Pulmonary Disease cases from Dongzhimen Hospital were studied.The Global Initiative for Chronic Obstructive Lung Disease 2019 report(GOLD 2019)and the "Guide to Traditional Chinese Medicine Diagnosis and Treatment of Chronic Obstructive Pulmonary Disease" were used as the guidelines for diagnostic criteria of stable COPD with ’Defiency of Lung and Spleen’ syndrome.Randomized grouping was used to determine the treatment and the control group.The control group was treated with conventional western medicine.The treatment group was given the ’Shu Yu Wan’ herbal paste,once a day,10g each time for three months.The conditions before and after treatment were compared between both groups,to evaluate the curative effect of ’Shu Yu Wan’ herbal paste.Outcome measures were:① Lung function(FEV1,FVC,FEV1/FVC);② The number of common colds and acute exacerbations;③ Clinical symptoms evaluation with CAT,mMRC,CCQ,Clinical Symptoms and Signs score table.[Results]63 cases who fulfilled the diagnostic criteria were recruited initially.Three cases were excluded and a total of 60 cases were included in the study.There were both 30 patients in the treatment group and the control group.No statistical difference between the two groups in the pre-treatment data was identified.The number of common colds,CCQ,and clinical symptom scores during the observation period after treatment in the two groups were statistically different from those before treatment(p<0.05).Values of FEV1/FVC,the number of colds,CAT,mMRC,and CCQ of treatment group after treatment were better compared to the control group.[Conclusion]1.The combination of conventional western medicine and ’Shu Yu Wan’ herbal paste improves clinical symptoms and quality of life of stable COPD patients more significantly,compared to those only received conventional western medicine.2.Although the improvements in FEV1 values of the treatment group were not significant,the values of FEV1/FVC values were significantly improved compared to the patients who only received conventional western medicine.3.Application of the theory of ’Preventive Treatment of Diseases’ as guidance of management for patients with stable COPD is scientifically feasible,and is worthy of clinical promotion and further research. |
