Study On The Effect Of The Method Of Invigorating The Kidney And Soothing The Liver On Hyperandrogenemia In Patients With Polycystic Ovary Syndrome

Objective1 Systematic evaluation of the efficacy and safety of Bushen Shugan method in the treatment of polycystic ovarian syndrome with hyperandrogenemia,to provide effective evidence for the clinical treatment of polycystic ovary syndrome with hyperandrogenemia.2 By observing the effect of Bushen Shugan method on TCM symptoms,clinical signs(hirsutism?acne?BMI),ultrasound scores and laboratory tests(T,E2,LH,FSH,LH/FSH,FPG,FINS)of patients with hyperandrogenemia of polycystic ovary syndrome of kidney deficiency and liver depression type,to explore the clinical effect of Bushen Shugan method on hyperandrogenemia of polycystic ovary syndrome of kidney deficiency and liver depression.Method1 The clinical randomized controlled trial on the treatment of polycystic ovary syndrome with hyperandrogenemia by the method of Bushen Shugan from the database of Zhiwang,Weipu,Wanfang and PubMed to January 2020,the process was carried out by tworesearchers alone,The literatures that met the inclusion criteria were evaluated by methodology,and the data were analyzed by Revman 5.3 software.2 Using self-contrast test,Wo collected 35 cases of hyperandrogenemia of polycystic ovary syndrome(37 cases in total,of which 1 case was lost,1 case is under treatment).The patients with kidney deficiency and liver depression,who were treated with Bushen Shugan method for 3 month.The patients' hirsutism,acne,ultrasound and TCM symptoms scores by scales.The scores of TCM symptoms before and after treatment were compared;scores of clinical signs such as hirsuteum,acne and BMI were compared before and after treatment;ultrasound scores were compared before and after treatment;the levels of T,E2,LH,FSH,LH/FSH,FPG,FINS and other laboratory indicators were compared before and after treatment;observe the curative effect of TCM symptoms and comprehensive curative effect.Result1 In the analysis of the effectiveness of Bushen Shugan method in the treatment of polycystic ovary syndrome,a total of 32 articles were retrieved,and the final 4 articles were included in the study,with 216 cases.The statistical results showed that the total clinical effective rate(OR=2.45,95%CI=1.20-4.99,P=0.01),se-rum sex hormone E2(MD=8.73,95%CI=3.15-14.30,P=0.002)in the intervention group were significantly higher than those in the western medicine intervention group;serum sex hormone LH/FSH(MD=-0.36,95%CI=-0.62--0.09,P=0.008),ovarian volume score(MD=-0.54,95%CI=-0.85--0.23,P=0.0006),acne score MD=-0.21,95%CI=-0.39--0.02,P=0.03),BMI value(MD=1.04,95%CI=0.13-1.94,P=0.03)were significantly lower than those in western medicine intervention group;the symptoms of traditional Chinese medicine were significantly improved compared with the intervention group(MD=-2.49,95%CI=-3.84--1.14,P=0.0003);there was no significant difference between the intervention group and the western medicine group in the levels of serum sex hormone T,LH,FSH and INS.2 Clinical study on the treatment of polycystic ovary syndrome hyperandrogenemia with Bushen Shugan method(1)Serum hormone levels:The method of Bushen Shugan can reduce the level of serum sex hormone T,LH and LH/FSH,the difference is statistically significant(P<0.01);increase the level of serum sex hormone E2,the difference is statistically significant(P<0.05);serum sex hormone FSH is slightly higher than before,but the difference is not statistically significant(P>0.05).(2)FINS FPG:The method of Bushen Shugan can reduce the level of FINS,the difference is statistically significant(P<0.01);the level of FPG is slightly lower than before,but the difference is not statistically significant(P>0.05).(3)Hirsute,Acne,BMI:The method of invigorating the kidney and soothing the liver can obviously improve the acne symptoms of the patients,the difference was statistically significant(P<0.01);the hairy symptoms and BMI were slightly improved,but the difference was not statistically significant(P>0.05).(4)Ultrasound scores:After the treatment of Bushen Shugan method,the ultrasonic score of the patients decreased slightly,but the difference was not statistically significant(P>0.05).(5)TCM syndrome score:The method of Bushen Shugan can obviously reduce the syndrome score of the patients(P<0.01).(6)Curative effect of TCM symptoms,The comprehensive curative effect:Total effective rate of TCM Syndrome of Bushen Shugan method in the treatment of hyperandrogenemia of polycystic ovary syndrome was 91.43%,The total effective rate was 88.57%,which was significant.Results1 In the systematic review,the results showed that compared with western medicine,Bushen Shugan Chinese medicine can effectively improve the clinical symptoms of polycystic ovary syndrome patients with hyperandrogenemia,reduce the syndrome score of traditional Chinese medicine,and improve the total effective rate of clinical treatment.However,due to the small quantity and low quality of the literature,and the lack of strong evidence,the research results need to be further verified in the RCT of high quality and large sample.2 In the clinical studies,the results showed that the method of Bushen Shugan can obviously improve the levels of serum sex hormone T,LH,E2 and LH/FSH in patients with hyperandrogenism of polycystic ovary syndrome of kidney deficiency and liver depression type,but it has no obvious effect on the level of ser-um sex hormone FSH,in a-ddition,it can also reduce the level of FINS,improve acne and reduce the TCM syndrome score.The total clinical efficiency is high and the effect is clear,which provides an effective TCM treatment method and thinking for the clinical treatment of Hyperandrogenemia of polycystic ovary syndrome.
