Yu Genchu's "Tongsu Treatise On Febrile Diseases", A Study On The Syndrome And Treatment Of Jueyin Disease

Objective:Yu Genchu is a representative doctor who advocates the combination of cold and warm.This study focuses on the relevant words and contents of Jueyin disease in Yu Genchu's Tong Su Shang Han Lun,summarizes the clinical experience and academic thought of Yu's diagnosis and treatment of Jueyin disease,clarifies Yu's understanding of the essence,pathogenesis and treatment for Jueyin disease,traces the ideological source of Yu's treatise on treating Jueyin disease by cold and warm combination,and explores the characteristics and innovation of Yu's medication.Methods:This study mainly focuses on "Chong Ding Tong Su Shang Han Lun".1.Through using the method of literature analysis,this study collects the contents of Jueyin disease in Huangdi's Classic on Medicine,Treatise on Febrile Diseases,Synopsis of the Golden Chamber,sorts out the discussion about the essence of the six channels and the diagnosis and treatment of Jueyin disease of Tao Jie'an,Zhang Jingyue and Ke Qin,restores the background of Yu's proposition of the six channels system and the combination of cold and warm,provides support for the analysis of Yu's thought of syndrome differentiation and treatment for Jueyin disease.This study also collects clinical reports to make clear the clinical application of Yu's prescriptions for Jueyin disease.2.Through using inductive summary method,this study analyses Shang Han Yao Yi,Shang Han Zhen Fa,Shang Han Mai She and other chapters,sums up Yu's understanding of the essence,pathogenesis,law of changes and diagnosis and other aspects of Jueyin disease.Through the analysis of the prescriptions put forward by Yu Genchu in the chapters of Liu Jing Fang Yao,Shang Han Ben Zheng,Shang Han Jian Zheng and Shang Han Jia Zheng for the treatment of Jueyin disease and the application examples of the prescriptions in specific diseases,this study summarizes the characteristics and experience of the treatment for Jueyin disease by Yu Genchu.3.Through using comparative study method,this study compares Yu's discussion on Jueyin disease with Huangdi's Classic on Medicine,Treatise on Febrile Diseases and Synopsis of the Golden Chamber,especially with the concept,pathogenesis and treatment methods of Jueyin disease in Treatise on Febrile Diseases,to explore the relationship and difference between them.By comparing the composition and dosage of Yu's modified prescriptions with those of the original prescriptions,this study analyzes Yu Genchu's understanding and development on the pathogenesis and treatment of Jueyin disease,combining with the specific application of the modified prescriptions.Results:1.Academic origin:Huangdi's Classic on Medicine,Treatise on Febrile Diseases and Synopsis of the Golden Chamber discussed Jueyin diseases from the internal organs,channels and function,mentioned the relationship between Jueyin and collaterals,and the treatment of Jueyin and collaterals diseases.Tao Jie'an advocated that the treatment of cold reduced disease should be different from that of warm disease.Zhang Jingyue proposed that the theory of six channels in Treatise on Febrile Diseases should be used to cover all exogenous febrile diseases.According to Ke Qin,the six channels are set up for all kinds of diseases,and the six channels ' disease syndromes are implemented to the body parts by the "six channels ground theory".These views have a great influence on Yu Genchu and are reflected in his works.2.Essence of Jueyin disease:Yu Genchu thought that the essence of the six channels is the sum of the functions of the internal organs and channels,connected six channels with human body structure,channels,internal organs and functions by pattern levels of six channels theory.The essence of Jueyin disease mentioned by Yu is the pathological changes on Jueyin meridian and viscera of hand and foot,including exogenous febrile diseases and miscellaneous diseases,but Tong Su Shang Han Lun mainly deals with exogenous febrile diseases.Jueyin disease discussed in Huangdi's Classic on Medicine and Treatise on Febrile Diseases focuses on foot Jueyin disease,while Yu discusses hand Jueyin disease a lot,reflecting the importance of hand Jueyin disease in exogenous febrile diseases.The exogenous Jueyin disease is mainly caused by the introduction of pathogenic heat into Jueyin,and there are two kinds of changes:heat and cold-heat complex.Jueyin is related to Chong channel,Ren channel,pericardium and liver collaterals.Delirium and spasticity in the extreme stage of exogenous fever are mostly related to hand Jueyin(pericardium)lesions,which are also related to foot Jueyin(liver).The main pulse of Jueyin disease is uneven pulse,which reveals that Jueyin disease is prone to yin deficiency and collaterals blockage.The tongue picture of Jueyin disease can change the shape,nature and color of tongue,which have corresponding clinical significance.The characteristic content of abdominal diagnosis of Jueyin disease is to examine the Chong Ren channel according to the pulsation in umbilical region.3.Treatment and prescription for Jueyin disease:Yu Genchu inherited the therapeutic methods and prescriptions of Jueyin disease chapter in Treatise on Febrile Diseases,and improved them with his own clinical experience.Yu believed that Jueyin was mainly characterized by wind and related to Shaoyang,so Jueyin disease always present as heat syndrome.He proposed that heat-clearing method should be the main treatment for Jueyin disease,and created and reformed a series of related prescriptions.Based on the physiological characteristics of Jueyin channels and viscera and the common pathological products,Yu paid attention to the coordination of regulating Qi movement,nourishing Yin and resolving phlegm while using heat-clearing method.For pericardium collaterals disease,Yu drew lessons from the syndrome differentiation theory and treatment methods of warm disease,combined with the clinical characteristics of pericardium disease,took clearing and dispelling heat of pericardium as the main treatment method,paid attention to opening Qi movement,resolving phlegm and stagnated blood,promoting urination.For liver collateral diseases,Yu made some explorations on the basis of Ye Tianshi's experience in using Xuanfuhua Decoction.For the deficiency syndrome of Jueyin,Yu's thought is to nourish Yin and subdue Yang.In terms of medication,the characteristics of Yu Genchu are to pay attention to cleaning heat and nourishing Yin,to use sweet and cold herb more,to pay attention to herb preparation and decoction,and to use local fresh herbs.In clinical practice.Yu Genchu carefully explored the causes and pathogenesis,and the treatment steps were orderly,which are worth learning for clinical doctors.Conclusion:1.On the basis of predecessors' viewpoints,Yu Genchu understood the essence of the six channels with internal organs,channels and function,and created "pattern levels of six channels theory" to guide the differentiation and treatment of diseases.His academic thoughts are in the same line with Huangdi's Classic on Medicine,Treatise on Febrile Diseases and Synopsis of the Golden Chamber.2.Yu Genchu's combined cold and warm,drew lessons from warm disease,added the content of Jueyin disease,especially hand Jueyin disease,improved the syndrome differentiation system of six channels,and demonstrated the combination of cold and warm.Some prescriptions formulated by Yu for Jueyin disease have high clinical practical value in the treatment of miscellaneous diseases.3.In order to get correct and valuable conclusion,we should restore the background of the prescription and summarize the application of the prescription in the medical cases.
