Observation Of Curative Effect Of Single-arm Clinical Trial Of Combined Traditional Chinese And Western Medicine For Limited-stage Small Cell Lung Cancer

Objection:To observe the non-progressive survival status and life quality changes of outpatients from China Academy of Chinese Medical Sciences Guang'anmen Hospital Oncology Department with local limited Small Cell Lung Cancer(SCLC),and to explore whether the combination of traditional Chinese medicine and Western medicine treatment can delay the progress of these patients,and to analyze the main influencing factors.Method:This study was an one-arm observational study of clinical cases,without setting a control group,and prospective observation method was used.Through collecting the outpatient cases of Guang'anmen Hospital with local limited SCLC treated by Pro.Zhang Peitong,we record their general information and the pathogenesis and treatment of tumor,and following their survivals and evaluated their life quality changes by delivering electronic questionnaire,as well as calculating the median progression-free survival,comparing with reported results so as to analyze prognostic factors.Result:Survival and Survival rate:Until March 15th,2020,the median progression-free Survival(PFS)of 84 patients with local limited small-cell lung cancer included in this study was 44.53 months,and median Overall Survival(mOS)was not yet estimated.The 1-year,2-year and 3-year progression-free survival rates were 59%,53%and 44%,respectively.The 1-year,2-year and 3-year overall survival rates were 92%,84%and 78%,respectively.Univariate analysis:the results showed that age,treatment time of TCM were the common influencing factors of PFS and OS,with significant correlation(P<0.05).Three factors like gender,central lung cancer/peripheral lung cancer,first-line treatment effect were the influencing factors of PFS,with significant correlation(P<0.05).However,as the difference of 11 factors including KPS score,smoking history,left lung/right lung,lymph node metastasis,staging,and a small amount of pleural effusion,surgery,chemotherapy cycles,the chest radiotherapy and sequential/synchronization,Prophylactic Cranial Irradiation(PCI)were not statistically significant(P>0.05),they could not be indicated as influencing factors of PFS and OS(P>0.05).COX multivariate analysis:Aging,central lung cancer/peripheral lung cancer,and duration for traditional Chinese medicine treatment were independent prognostic factors for progression-free survival of local limited SCLC.Age is an independent prognostic factor for the overall survival of local limited SCLC.Female was probably the protective factor in progression-free survival,with a 0.38 times greater risk of progression than male,while the difference was not statistically significant(P=0.06).Aging?60 years was a risk factor for progression-free survival,and Relative Risk(RR)of progression was 3.7 when compared with patients<60 years(P<0.05).Peripheral lung cancer is a protective factor in progression-free survival,and the risk of progression is 0.37 times that of patients with central lung cancer.Compared with taking Chinese medicine treatment for more than 36m,duration for 6?12m was a risk factor for progression-free survival with RR=3.76,with significant correlation(P<0.05).Age?60 years was a risk factor for overall survival,with the risk of death 16.523 times that of patients younger than 60 years.Quality of life:The patients' life quality was basically stable,and the KPS score of the patients was improved with the combination of Chinese and western treatment,and the difference was statistically significant.(P<0.05)Conclusion:The results of this study suggests that the combination of Chinese and western medicine treatment has a protective effect on progression-free survival of patients with local limited small-cell lung cancer.Preliminary results showed that The best curative effect was taking Chinese medicine treatment for 24?36 months.
