| ObjectiveTo apply Zhuyu Huoxue decoction in clinic,through the collection and arrangement of observation indexes,to observe the therapeutic effect of postoperative swelling of calcaneal fracture,and to analyze it by statistical method.Objective to evaluate the safety and efficacy of Zhuyu Huoxue decoction in the treatment of postoperative swelling of calcaneal fracture.Methods60 patients with soft tissue swelling after single limb closed fresh calcaneal fracture treated in Henan Hospital of traditional Chinese Medicine from January 2020 to December 2020 were randomly divided into observation group(n=30)and control group(n=30).The patients in both groups were treated with open reduction and internal fixation of calcaneal fracture with lateral “L” incision and plate internal fixation by doctors with the same professional title with many years’ experience.All patients followed the PRICE principle and were given sodium aescin injection for detumescence treatment,while the treatment group was treated with Zhuyu Huoxue decoction.Develop a unified case questionnaire to extract the required information:patient demographic data;for patients who meet the inclusion criteria and complete data,the circumference difference of swelling on the 1th,3th,5th,7th and 10 th day after operation,the time of the first appearance of dermatoglyphics,the time of incision healing and skin complications were collected,and the collected data were compared between the two groups at the same time.SPSS26.0 statistical software was used for statistical analysis to objectively evaluate the safety and efficacy of Zhuyu Huoxue decoction in the treatment of postoperative swelling of calcaneal fracture.Results(1)Comparison of general data: there was no significant difference in sex,age,location of fracture,classification of fracture,duration of operation,degree of postoperative soft tissue swelling and drainage between the two groups(P>0.05).The two groups are comparable.(2)Comparison of observational indexes: 1.The average occurrence time of the first dermatoglyphics in the treatment group was5.07 ± 0.45 days,which was earlier than that in the control grou(5.77 ± 0.57days).There was significant difference between the two groups(P<0.05).2.Comparison of incision healing time: the average postoperative wound healing time in the treatment group was15.90±0.71 days,which was earlier than that in the control group(17.20 ± 0.71days).There was significant difference between the two groups(P<0.05).3.Comparison of swelling circumference difference at 1th,3th,5th,7th and 10 th days after operation: on the 1th and 3th day after treatment,the swelling circumference difference between the two groups was tested by independent sample t-test,and the results showed that there was no statistical significance(P>0.05).On the 5th,7th and 10 th day,the independent sample t-test was performed between the two groups,and the results showed that the swelling circumference difference between the two groups was statistically significant(P<0.05).After repeated measurement anova,without considering grouping effect,the peripheral diameter difference of postoperative swelling in all 60 patients changed with time(F=529.600,P<0.05).The results of group and time effect showed P<0.05,indicating that time and group had interaction,and with the change of time,the effect of swelling reduction in the experimental group and the control group also changed differently,and the degree of swelling subsidence in the two groups was different.4.Comparison of the incidence of complications: there were 3 cases of tension blisters in the treatment group,and the complication rate was 10%.In the control group,there were 3 cases of tension blisters,3 cases of incision skin margin blackened,and the incidence of complications was 20%.There was no statistical significance between the two groups(P>0.05).5.Comparison of swelling elimination rate: in the treatment group,2 cases were cured,23 cases were markedly effective,4 cases were effective,1case was ineffective,the effective rate was 96.67%;in the control group,1 case was cured,12 cases were markedly effective,15 cases were effective,2 cases were ineffective,the effective rate was 93.33%.The swelling elimination rate of the two groups was statistically significant(P<0.05).(3)Comparison of safety indicators: 1.In terms of ALT,the intra-group comparison between the two groups before and after treatment showed P>0.05,and the comparison between the two groups at the same time point before and after treatment showed P>0.05,both of which showed no statistical significance.2.In terms of AST,P>0.05 was found between the two groups before and after treatment,and P>0.05 was found between the two groups at the same time point before and after treatment,both of which showed no statistical significance.3.In the aspect of Cr,the intra-group comparison of the two groups before and after treatment showed P>0.05,and the inter-group comparison of the two groups at the same time point before and after treatment showed P>0.05,both of which showed no statistical significance.It shows that Zhuyu Huoxue Decoction can be used safely.Conclusion:1.Zhuyu Huoxue decoction can effectively reduce the postoperative soft tissue swelling of calcaneal fracture.Compared with the control group,the time of detumescence is shorter and the effect is more obvious.It is an effective treatment.2.Zhuyu Huoxue decoction can effectively promote incision healing,compared with the control group,incision healing time is earlier.3.Zhuyu Huoxue decoction is safe and effective in clinical observation,and no toxic side effects are found,so it is suitable for clinical application. |
