TaoHongXiao Yu Decoction For The Treatment Of Tibial Plateau Fracture Perioperative Swelling And Venous Thrombosis Of The Clinical Observation And Research

Objective:To observe the curative effect of taohong xiaoyu decoction on perioperative swelling and deep venous thrombosis of tibial plateau fracture by comparing it with conventional methods of detumescence and analgesia.To explore the influence of "taohong xiaoyu decoction" on the incidence of deep venous thrombosis in lower limbs of perioperative patients with tibial plateau fracture;To discuss the mechanism of prevention and treatment of perioperative swelling and deep venous thrombosis of tibial plateau fracture.Methods:All the inpatients admitted to the microorthopedics ward of the affiliated hospital of shandong university of traditional Chinese medicine from October 2016 to January 2018 were included in this study.All the subjects were patients with tibial plateau fracture.A total of 50 patients with "tibial plateau fracture" who met the inclusion,exclusion and termination criteria were selected strictly according to the criteria established in this study.The patients were divided into two groups: the experimental group and the blank control group.Both groups received conventional treatment after surgery.On this basis,the Chinese medicine treatment group was given taohong xiaoyu decoction by mouth,while the blank group received conventional detumescence and analgesia treatment.1.Venous blood was drawn on an fasting basis on the second day after admission,the second day after surgery,and 1 week after surgery,and was sent to the laboratory of our hospital for analysis.Blood routine,liver function,d-dimer(d-dimer)gold standard method,partial thrombin activation time(APTT),prothrombin time(PT),and fibrinogen(FIB)were detected.2.On the second day after admission and after surgery,if LDVT symptoms and signs occur in the subjects,bilateral lower extremity arteriovenous ultrasonography should be performed immediately to record the incidence.3.The day after admission,the second day after surgery,and 1 week after surgery,the circumference of the affected limb was measured;4.Patients in the two groups were evaluated and recorded by VAS pain score on the second day after admission,the second day after surgery,and the week after surgery.Results:1.Two groups of patients in postoperative week,the second day after operation,preoperative VAS score were statistically significant(P < 0.05),through the observation data can be found at the same time,Chinese traditional medicine group in patients with lower perioperative VAS score better than blank control group,which to a certain extent,suggests Chinese medicine group in perioperative analgesic action.2.All the patients were measured at the knee midpoint,the upper knee,and the lower knee 10 cm circumference before the surgery,the second day after the surgery,and the week after the surgery,respectively.After statistical examination,the overall difference between the two groups was statistically significant(P<0.05).This suggests that the experimental group of traditional Chinese medicine is superior to the blank control group in alleviating the swelling of the affected limb,which also indicates that taohong xiaoyu decoction has certain detumescence effect during the perioperative period.3.There was no significant difference in PT test results between the two groups at different time points(P=0.10,P=0.2,P=0.211);There was no statistically significant difference in d-dimer between the two groups before surgery and the second day after surgery(P=0.63,P=0.99),while there was statistically significant difference in d-dimer between the two groups one week after surgery(P=0.056).There was no statistically significant difference in APTT between the two groups before surgery(P=0.49),while there was statistically significant difference in APTT between the two groups on the second day and one week after surgery(P=0.02,P=0.015).It can be found from the above results that the traditional Chinese medicine experimental group can only have a limited effect on improving the prethrombotic state of patients,which also preliminarily suggests that compared with the blank control group,taohong xiaoyu decoction can only have a certain effect on improving postoperative APTT,d-dimer and other indicators one week after the operation.4.The data of WBC,ALT and AST were tested at different times between the two groups,and the statistical test proved that there was no statistical significance between the two groups,which also showed that taohong xiaoyu decoction could prevent the swelling of the affected limb and the deep vein thrombosis of the lower limb without causing the problem of drug safety.Conclusion:Through clinical trials,according to the results of peach eliminating stasis soup in the prevention and treatment of tibial plateau fractures in patients with perioperative limb swelling can relieve the role of,can effectively improve perioperative state before part of thrombus,to reduce the tibial platform at the same time a perioperative incidence of lower extremity deep vein thrombosis has good effect,make the patient to avoid complications,shorten the treatment time,accelerates the pace of recovery,has the clinical significance.
