| Objective: To observe the clinical symptoms,ulcer healing effect under gastroscope,eradication rate of Helicobacter pylori and ulcer recurrence rate of Shugan Yuyang Decoction combined with western medicine in the treatment of peptic ulcer of liver-stomach stagnant heat type,so as to objectively evaluate the clinical efficacy and safety of Shugan Yuyang Decoction combined with western medicine in the treatment of peptic ulcer,and to explore its mechanism of action in the treatment of peptic ulcer from two perspectives: modern medicine and traditional medicine.Methods: The data of patients with active peptic ulcer(PU),epigastric pain and liver-stomach stagnant heat syndrome were collected from October 2018 to September 2019 in the Outpatient Department of Director Yesong,Hubei Provincial Hospital of Traditional Chinese Medicine.According to the inclusion and exclusion criteria,a total of 80 PU patients who finally met the criteria of this study were numbered according to the order of presentation and divided into the treatment control group by the random number method.The control group was treated with pantoprazole,amoxicillin + clarithromycin if Hp infection occurred,and the control group was treated with Shugan Yuyang Decoction.The total course of treatment was 8 weeks,and endoscopy was reexamined.During this period,various observation indicators of patients were objectively recorded.All and single quantitative scores before treatment and after the end of treatment were compared between the two groups,and all and single quantitative scores after treatment were compared between the two groups to understand the changes in the patient’s condition;the degree of endoscopic ulcer healing and Helicobacter pylori eradication rate before and after treatment were compared;the ulcer recurrence rate was compared between the two groups 3 months after the end of treatment by telephone,in order to evaluate the clinical efficacy of Shugan Yuyang Decoction.Results: 1.All clinical quantitative scores: After t test,there were significant differences in all clinical quantitative scores before and after treatment between the two groups(P<0.01).The symptoms of the two groups were improved after treatment,and the treatment group was more obvious.It may be related to the effect of reducing gastric acid secretion,relieving pain and promoting ulcer repair.2.Single clinical quantitative score: The single clinical quantitative score of patients in each group was compared before and after treatment,and the single clinical quantitative score of patients in the two groups after treatment was compared.The results were statistically significant by t-test(P<0.05);while the change of dry mouth symptoms in the treatment group was superior to that in the control group(P<0.05),and the improvement of symptoms of two flank fullness,irritability,epigastric pain,pantothenic acid,and bitter taste was significantly superior to that in the control group(0.01<P<0.05);in terms of constipation symptoms,the comparison results between the two groups were not statistically significant(P>0.05).The reason for this may be related to the fact that some elderly subjects also have deficiency of qi and yin.3.Clinical recovery rate: 77.5% of the 40 patients in the treatment group were clinically recovered,of which clinical recovery: significantly effective: ineffective = 17:14:5: 4;while60% of the 40 patients in the control group were clinically recovered,of which clinical recovery: significantly effective:ineffective = 8:16:10:6.After rank sum test,there was significant difference in the clinical recovery rate between the two groups(P< 0.05).The recovery rate in the treatment group was significantly higher than that in the control group,indicating that the treatment group had better efficacy.4.Ulcer healing rate: 72.5% of the 40 patients in the treatment group had ulcer healing,of which clinical recovery: significantly effective: effective = 16:13:8: 3;while 62.5% of the 40 patients in the control group had ulcer healing,of which clinical recovery:significantly effective: effective = 7:18:6: 9.By rank sum test,there was significant difference between the two groups(P<0.05).The ulcer healing rate of the treatment group was better than that of the control group,indicating that the treatment group was superior to the control group in promoting ulcer healing.5,H.pylori eradication rate: 31 patients in the treatment group were negative for H.pylori after treatment: the positive rate was 25:6,and the eradication rate was 81%.In the control group,28 patients were negative for Helicobacter pylori after treatment:positive was 15:13,and eradication was 54%.By Chi-square test,there was significant difference between the treatment group and the control group(P < 0.05),indicating that the eradication rate of the treatment group was significantly higher than that of the control group6.Ulcer recurrence rate: The ulcer recurrence rate of 40 patients in the treatment group was 15% after treatment,including no recurrence: recurrence = 34:6;the ulcer recurrence rate of the control group was 35%,including no recurrence: recurrence = 26:14.By chi-square test,the difference between the two groups of data was statistically significant,and the ulcer recurrence rate in the treatment group was lower than that in the control group.7.No obvious drug allergy and adverse reactions occurred in the two groups during the treatment,and no obvious abnormalities were found in the relevant examinations,indicating that the treatment regimens of the two groups were safe.Conclusion: Shugan Yuyang Decoction combined with western medicine is effective in treating peptic ulcer of liver-stomach stagnant heat type,and is superior to the control group in improving clinical symptoms,promoting the healing of ulcer surface,improving the eradication rate of Helicobacter pylori and reducing the recurrence rate of ulcer.No obvious adverse drug reactions were found during treatment,which is worthy of further study and clinical application. |
