| Objective:To observe the effect of unitary acupoint application on abnormal blood pressure circadian rhythm in patients with non-dipper hypertension, and to provide new ideas for clinical diagnosis and treatment of abnormal blood pressure rhythm in patients with hypertension. To explore the feasibility of treating circadian rhythm disorder of blood pressure in patients with non-dipper hypertension by time acupoint application. Methods:A total of 80 subjects with grade 1 and 2 primary hypertension(yin deficiency and yang hyperactivity syndrome)were randomly divided into control group(n=40)and experimental group(n=40).Two groups of patients were given acupoint application, the control group(8:00~10:00)application,the experimental group(17:00~19:00)application,once a day,4 hours each time, continuous intervention for 4 weeks. The blood pressure,24-hour ambulatory blood pressure,TCM syndrome score and other indicators of the two groups were observed to evaluate the improvement of blood pressure and syndrome.Result:75 cases were collected, including 37 cases and 3 cases in the experimental group; In the control group,38 cases were completed and 2 cases fell off.1.Comparison of baseline data:Before treatment, the clinical data of two groups were analyzed and there was no significant difference(P>0.05).Before treatment, there were no significant differences in the blood pressure and ambulatory blood pressure values, total scores of TCM syndromes and single TCM syndromes between the two groups(P>0.05).2.Comparison of 24-hour ambulatory blood pressure:After the intervention, the nighttime mean diastolic blood pressure, daytime mean systolic blood pressure and diastolic blood pressure,24h mean systolic blood pressure and diastolic blood pressure of the control group were significantly lower than those before the intervention(P<0.05),but there was no statistical difference in nighttime systolic blood pressure of the control group after the intervention(P>0.05);Compared with the control group before and after the experiment, the nighttime mean systolic and diastolic blood pressure, daytime mean systolic and diastolic blood pressure,24h mean systolic and diastolic blood pressure decreased significantly(P<0.05),while the daytime diastolic blood pressure and 24h mean diastolic blood pressure had no significant difference(P>0.05);Compared with the control group, the nighttime mean systolic blood pressure and diastolic blood pressure, daytime mean systolic blood pressure and 24h mean systolic blood pressure in the experimental group decreased significantly(P<0.05),while the daytime diastolic blood pressure and 24h mean diastolic blood pressure had no significant difference(P>0.05).3.Comparison of curative effects of TCM syndromes:before and after intervention, there were significant differences in vertigo and headache in the control group(P<0.05),but there were no significant differences in symptoms such as backache, soft knees, vexation and palpitation(P>0.05).There are significant differences in vertigo, headache, vexation and insomnia in the experimental group(P<0.05),but no significant differences in soreness of waist, weakness of knees, palpitation, tinnitus and forgetfulness(P>0.05).Compared with the single TCM syndrome of the two groups, the scores of vertigo, headache, vexation and insomnia in the experimental group decreased significantly(P<0.05),but there was no significant difference between the two groups in soreness of waist, palpitation, tinnitus, forgetfulness and tongue coating(P>0.05).Conclusion:1.Time-hour acupoint application can assist in reducing nocturnal blood pressure of non-dipping type hypertension patients, improve dizziness, headache, waist acid and other symptoms in patients with Yin deficiency and Yang hyperactivity syndrome, and indirectly improve the quality of life of patients.2.The effect of hourly acupoint application in improving the nocturnal blood pressure and clinical syndrome of patients with non-dipper type hypertension is better than that of conventional acupoint application in conventional period, reflecting the nursing principle of "adapting to the time conditions" in Chinese medicine. |
