Research On Medication Regularity Of Traditional Chinese Medicine Treatment Of Uric Acid Nephropathy Based On Data Mining

Purpose:This study sorts out the randomized controlled trials of oral Chinese medicine compound in the treatment of Uric Acid Nephropathy in the past 20 years,screens and counts the prescriptions of Chinese medicine,and uses the "Chinese Medicine Inheritance Assistant System,to study and summarize the characteristics of oral Chinese medicine compound prescriptions in the treatment of uric acid nephropathy.Compare the medication rules in the past ten years,coastal and inland,to provide clinical reference.Material and method:A computer search was published in CNKI,WF,and VIP databases between January 1,2000 and December 31,2020 about the treatment of uric acid nephropathy by oral Chinese medicine compound.According to the inclusion and exclusion criteria of the literature,the collected literature is screened and checked and the included literature is finally confirmed.Extract the basic information and Chinese medicine prescription data included in the literature,unify and standardize the data and establish a database,enter the Chinese medicine prescription into the Chinese Medicine Inheritance Assistant System,and apply frequency statistics,association rules,improved mutual information method,complex system entropy clustering and other methods.Data mining is used to obtain data results such as the frequency,drug properties,meridian tropism,main efficacy,the analysis of the prescription law,high-frequency drug pairs,core combinations,and new prescriptions.Grouped by time and region,statistical analysis and comparison of high-frequency drugs,drug properties,meridian tropism,main efficacy of 2000-2010,2010-2020,and coastal group and inland group were carried out.Results:1 In this study,96 Chinese medicine prescriptions were collected,involving 142 Chinese medicines,with a cumulative frequency of 1,133 medicines.There are 24 traditional Chinese medicines with frequency of medication≥15 times.The top ten high-frequency medicines are Smilax glabra Roxb,Coix seed,Bixie,Astragalus,Achyranthes,Atractylodes,Clematis chinensis,Salvia miltiorrhiza,Rhubarb,and Phellodendron amurense.The four qi medicines are mainly warm,flat and cold medicines,the five flavors are mainly sweet,bitter and pungent medicines,and the main meridians are the liver meridian,spleen meridian and kidney meridian.The main efficacy of the drugs is tonic,diuresis and dampness,activating blood and removing blood stasis,clearing heat,and dispelling rheumatism.Among them,qi replenishing drugs,diuretic and swelling drugs,activating blood and regulating menstruation drugs,clearing heat and detoxifying drugs,and dispelling wind and cold dampness.The medicines accounted for the highest proportions of tonic medicines,diuretic and dampening medicines,blood activating and removing blood stasis medicines,heat clearing medicines,and rheumatism medicines,all of which accounted for more than 50%.Based on the association rules,when the adjustment support is 20%and the confidence is 0.8,39 groups of high-frequency drug pairs and 6 groups of drug combinations are obtained.Based on the complex system entropy clustering analysis method,when the correlation degree is adjusted to 7 and the penalty degree is 3,8 core groups and 4 new combinations are obtained.2 Comparison of medication usage in the past ten years:From 2000 to 2010,40 articles were included in the literature,involving 103 flavors of traditional Chinese medicine,with a cumulative frequency of 456 times.From 2010 to 2020,56 articles were included in the literature,involving 122 flavors of traditional Chinese medicine,with a cumulative frequency of 675 times.The top three high-frequency drugs from 2000 to 2010 were Coix seed,Bixie,and Astragalus,and The top three high-frequency drugs from 2010 to 2020,Smilax glabra Roxb,achyranthes,and coix seed.Among the top 10 high-frequency drugs,the same are Coix Seed,Bixie,Astragalus,Smilax glabra Roxb,Salvia miltiorrhiza,Rhubarb,Achyranthes,Atractylodes,Clematis chinensis,Plantago Seed.2000-2010 high-frequency drugs also include Salvia miltiorrhiza,Rhubarb,Plantago Seed;2010-2020 high-frequency drugs also includes Phellodendron amurense,Atractylodes,Alisma.There was no statistical difference between the two groups in the distribution of drug properties and meridian(P>0.05),and the main efficacy distribution was statistically different(P<0.001).3 Comparison of the law of drug use between coastal and inland areas:51 articles were included in coastal areas,involving 120 Chinese medicines,with a cumulative frequency of 591 times,and 45 articles were included in inland areas,involving 106 Chinese medicines,with a cumulative frequency of 542 times.The top three high-frequency drugs in coastal areas are coix seed,Smilax glabra Roxb,and Bixie,and the top three high-frequency drugs in inland areas are Smilax glabra Roxb,coix seed,and atractylodes.The same drugs in the top 10 high-frequency drugs include Coix Seed,Smilax glabra Roxb,Bixie,Achyranthes,Astragalus,Clematis chinensis,Atractylodes,and Salvia miltiorrhiza;the high-frequency drugs in coastal areas also include Rhubarb,Atractylodes,and the high-frequency drugs ininland areas also include Phellodendron,Cornus officinalis.There was no statistical difference between the two groups of drug in drug properties,meridian tropism and main efficacy distribution(P>0.05).Conclusion:1 Uric acid nephropathy is mostly manifested as a syndrome of deficiency and excess,based on deficiency of the spleen and kidney,and damp-heat phlegm and blood stasis as the standard.The disease is located in the kidney and is closely related to the liver,spleen and other organs.2 The overall treatment emphasizes equal emphasis on invigorating deficiency and eliminating evil,combining movement and static,regulating tonic,combining treatment of damp-heat and blood stasis,and coordinating liver,spleen and kidney.The main medicines are qi-tonifying drugs,diuretic and swelling drugs,blood-activating menstrual-regulating drugs,heat-clearing and detoxifying drugs,and anti-wind and cold dampness drugs.Most of them are sweet and warm products.The representative drugs are Smilax glabra Roxb,Coix seed,Bixie,Astragalus,Achyranthes,Atractylodes,Clematis chinensis,Salvia miltiorrhiza.etc.,and Astragalus as the center to form a core drug pair such as Smilax coix-coixen.3 In the past 20 years,the rule of medication for uric acid nephropathy has evolved:the treatment is still based on the fundamental method of replenishing the deficiency and reducing the actual condition,but tends to focus on eliminating pathogens and supplementing the deficiency,highlighting the early dialectical treatment of the disease,the internal syndrome of damp-heat.The syndrome of blood stasis blocking collaterals,phlegm turbidity and internal resistance are gradually increasing.Treatments such as clearing heat and removing dampness,diuresis and turbidity,promoting blood circulation and removing blood stasis,promoting qi and diuresis,and dispelling the evil toxins.4 There is no significant difference in the drug properties,meridian tropism and main effects of the drugs used in the inland and coastal areas.
