| ObjectiveThis subject uses a randomized controlled study to explore the clinical efficacy of Acupuncture Therapy of Dredging Governor Vessel for Regulating Mentality in vascular cognitive impairment no dementia and to provide a reference for clinical acupuncture treatment of this disease.MethodsA total of 82 cases were enrolled,all patients were selected from Guangdong Province Traditional Chinese Medical Hospital in July 2020 to February 2021.According to the random number table,they were randomized at a ratio of 1:1 assigned to the control group and the treatment group(Acupuncture Therapy of Dredging Governor Vessel for Regulating Mentality),41 cases in each group,The control group was given routine neurology treatment,and the treatment group was given acupuncture therapy of Dredging Governor Vessel for Regulating Mentality,main points:DU20,DU24,EX-HN3,DU26,RN24,RN23;matching points:deficiency of spleen and kidney with SP2,K 17,insufficient Qi and blood with RN6,ST36,phlegm blocking orifice match ST36 and ST40,blood stasis blocking the collaterals with BL17 and SP10,Yin deficiency and Yang hyperactivity match with BL18,BL23 and LR2,heat toxin is matched with HT8 and LR3,Fu staying in turbidity matches ST36 and RN12.Disposable acupuncture needles with a diameter of 0.25mm and a length of 25-40mm.Acupuncture is performed once a day,and 5 times is a course for a total of 2 courses.Both groups of patients received the Montreal Cognitive Assessment Scale(MoCA),the Mini Mental State Examination Scale(MMSE),and the Ability of Daily Living Scale(ADL)before treatment,after the first course of treatment,after the second course of treatment,and one month after the end of treatment.Finally,the data was statistically analyzed to evaluate the clinical efficacy of Acupuncture Therapy of Dredging Governor Vessel for Regulating Mentality in VCIND.Results(1)Baseline situationThe general baseline conditions of the two groups of patients,such as gender,age,course of disease,TCM syndrome differentiation,education level,and MoCA,MMSE,and ADL scale scores before treatment,were statistically tested,and there was no statistically significant difference between the groups(P>0.05),suggesting that the distribution of baseline characteristics of the two groups of patients is basically balanced and comparable.(2)curative effect analysis2.1 Cognitive function①Repeated measures analysis of variance was performed on the MoCA and MMSE scale scores of the two groups of patients at each observation point.The time multivariate test in the within-subject effect test indicated that the two groups of treatments can improve the cognitive function of VCIND patients(P<0.01).The difference between the MoCA and MMSE scale scores of the two groups of patients at each observation time point was compared between groups,which indicated that the MoCA and MMSE scale scores of the treatment group at each observation time point were better than those of the control group(P<0.01).②Comparison of the MoCA scale scores of the two groups of patients at each observation point:This data has 4 observation time points,5 pairwise comparisons are required,and the inspection level is adjusted a=0.05/5=0.01.After the first course of treatment,the scores of the MoCA scale of the two groups of patients increased compared with before treatment(P<0.01).After the second course of treatment,the scores of the MoCA scale of the patients in the treatment group were higher than before and after the first course of treatment(P<0.01).There was no significant difference between the control group and after the first course of treatment(P>0.01).There was still an improvement compared to before treatment(P<0.01).Follow-up one month after the treatment,the MoCA scale scores of the patients in the treatment group decreased compared with that after the second course of treatment(P<0.01).Compared with the second course of treatment,the scores of the control group had a downward trend,and the difference was not significant(P>0.01).The scores of the two groups were still improved compared with before treatment(P<0.01).It is suggested that in the two courses of treatment,the MoCA scale score of the treatment group was positively correlated with the treatment time,and the score decreased in the follow-up one month after the treatment.Although the MoCA scale score of the control group also increased to a certain extent,it increased with the course of the disease.The score has declined.③Comparison of the MMSE scale scores curative effect evaluation:The overall curative effect of the treatment group at each observation point was better than that of the control group(P<0.05).2.2 Ability of daily living①Repeated measures analysis of variance was performed on the ADL scale scores of the two groups of patients at each observation point.Among them,the time multivariate test in the within-subject effect test indicated that the two groups of treatments can improve the ability of daily living of VCIND patients(P<0.01).The difference between the ADL scale scores of the two groups of patients at each observation time point was compared between groups,which indicated that the ADL scale scores of the treatment group at each observation time point were better than those of the control group(P<0.05).②Comparison of the groups of patients had no significant difference compared with the second course of treatment(P>0.01),and were improved compared with before treatment(P<0.01).It is suggested that in the two courses of treatment,the ADL scale scores of the two groups of patients were positively correlated with the treatment time.Follow-up one month after the treatment ended,the scores of the two groups of patients decreased.ConclusionAcupuncture Therapy of Dredging Governor Vessel for Regulating Mentality can significantly improve the cognitive function and ability of daily living in patients with VCIND,and within two courses of treatment,the curative effect is positively correlated with the treatment time,and the follow-up curative effect decreased after one month of treatment;conventional neurology treatment can also improve the cognitive function and ability of daily living in patients with VCIND,but as the course of the disease grows,the efficacy has declined. |
