Influence Of Acupuncture Therapy Of Regulating Conception Vessel And Unblocking Governor Vessel On Cerebral Hemodynamics Changes In Patients With Mild Cognitive Impairment After Cerebral Infarction

ObjectiveThis research is to observe the clinical therapeutic efficacy of acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel in the treatment of mild cognitive impairment(MCI) after cerebral infarction,and detect the cerebral hemodynamic evaluated by Transcranial Doppler combined with breathless test, so to explore the therapeutic effect of acupuncture on cognitive function after stroke and its relationship with the change of the cerebral blood flow.MethodsIn total sixty cases of patients with mild cognitive impairment after stroke which were in accordance with the inclusive criteria were randomized into treatment group and control group. In the treatment group,acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel was applied; in the control group, routine acupuncture was used. The treatment lasted thirty minutes each time and carried once daily for continuous 21 days. Before the treatments, the Mini-Mental State Examination(MMSE),Montreal cognitive assessment(MoCA), the Barthel index of ADL(BI) were evaluated,and the both side of middle cerebral artery were detected by Transcranial Dop pler combined with breathless test, collect and calculate the systole Velo city (Vs), diastolic Velocity (Vd), Mean Velocity (Vm), Pulsatility Index (P I), Resistance Index (RI), Breathless Test Index (BHI). After the treatments, all the assessment and TCD tests would be detected again.Finally,all the data were compared and analyzed statistically.Results1. The change of neural function and the daily activities after the acupuncture therapy:the experimental group and control group before and after treatment MMSE scale score, MoCA scale, Barthel scale score comparison were improved (P<0.01); Comparison between the two groups after treatment, on the MMSE scale score, MoCA rating scale to improve significant differences (P<0.05), no significant differences in Barthel scale scores improved (P> 0.05).2.The impact of the acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel on cerebral hemodynamics:In the TCD inspection and breathless test, the experimental group and control group before and after treatment comparison Vm value and BHI index were improved (P<0.01,P< 0.05), Compared the two groups after treatment, the change of Vm was no significant difference (P> 0.05), BHI values were significantly different(P<0.05); before and after the experimental treatment comparison PI, RI index were significantly decreased (P<0.05),control group there was no significant change (P> 0.05).3. The relationship between the improvement of cognitive impairment and the change of cerebral vascular reactivity:The change of BHI score and the changes value of MMSE scale, MoCA scale score have significant positive correlation.Conclusion1. The acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel and common acupuncture method in the treatment of mild cognitive impairment after cerebral infarction curative effect affirmation, in improving the curative effect on daily activity ability approximation, but acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel on the improvement of the cognitive function is better than the contrast ordinary acupuncture group.2.Both the acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel and common acupuncture can improve cerebral infarction cerebral hemodynamics in patients with mild cognitive impairment, And the acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel on the improvement of cerebrovascular reactivity is better than ordinary acupuncture method.3. The change of MMSE and MoCA score there is significant correlation between the change of BHI value. The level of mild cognitive impairment after cerebral infarction and Cerebrovascular reactivity level are in correlation. This may be a potential mechanisms of acupuncture therapy of Regulating Conception Vessel and Unblocking Governor Vessel treatment of cognitive dysfunction.
