Clinical Experience Of Professor Fang Zhuyuan’s Treatment Of Coronary Heart Disease Angina Pectoris Based On Data Mining

Objective:The purpose of this project is to explore the experience and thought of Fang Zhuyuan in the treatment of "coronary heart disease" by means of the latest information technology data mining method,on the basis of Professor Fang Zhuyuan’s in the diagnosis and treatment of coronary heart disease.Methods:From June 2017 to June 2020 in Jiangsu Province Chinese Medicine Hospital’s outpatient department,we collected the cases of coronary heart disease patients which Professor Fang Zhuyuan recorded and a database was established.SPSS moldeler 18.0,SPSS Statistics 22.0 and ancient and modern medical case cloud platform software were used to conduct frequency analysis,association rule and cluster analysis on patients’ general information,tongue pulse,prescriptions and symptoms,and the results were derived in the form of charts.Results:In this study,13 patients with 309 consultations were enrolled,including 46 males(41%)and 67 females(59%).The youngest was 43 years old and the oldest was 96 years old.The age of concentrated distribution of CAD was between 50 and 79 years old(81.41%).59 patients(52.21%)were complicated with hypertension.Coronary artery stent implantation and coronary artery bypass grafting were performed in 26 patients(23.00%).There were 26 patients(23.00%)with abnormal glucose and lipid metabolism,including 15 patients with diabetes and 11 with hyperlipidemia.There were 18 patients(15.93%)with other heart and brain disease,12 patients with cerebral infarction,and 6 patients with atrial fibrillation.Among 116 clinical symptoms involved,chest tightness,poor sleep,chest pain,palpitation and dizziness were the most important symptoms(≥20%).Symptoms of digestive system such as poor digestion,loose stools,dry mouth and constipation were also common(14.53%).Eight syndrome elements were involved,referring to deficiency of Qi 53.07%,deficiency of Yin 15.53%and deficiency of Yang 0.97%.Stasis of Blood 82.52%,stagnation of Qi 45.63%,phlegm 10.03%,heat 1.29%,cold 0.97%.Fourteen kinds of tongue signs were obtained,including blood stasis related tongue signs(45.08%).Thermal-related tongue pattern(34.09%);Tongue image related to Qi deficiency(14.02%);Yin deficiency related tongue image(7.95%).According to the statistics of lichen quality,there were 101 times of thin lichen,50 times of thin and slightly greasy lichen,22 times of greasy lichen and 23 times of little lichen.There were 125 white coating times and 46 yellow coating times.Eleven kinds of pulse were obtained,among which chordal pulse,fine pulse,smooth pulse and sinking pulse were the main ones.A total of 184 kinds of drugs were used,including warm drugs(25.98%),cold drugs(37.74%),flat drugs(28.67%),cool drugs(7.34%)and hot drugs(0.27%).The medicinal taste is mainly sweet,bitter and pungent.Drug return to the heart meridian and lung meridian.Codonopsis and Salvia miltiorrhiza were the most frequently used drugs.The most high frequency drugs(>70%)were Salvia miltiorrhiza,Codonopsis Radix Codonopsis,Schisandrae schizandrae,Radix Curcumae,Lilium officinalis,Radix Ophiopogonis,Rhizoma Curcumae,Cortex Moutan,Rhodiola rhodiola,Astragalus membranaceus and Gynostemma pentaphyllum.Commonly used for the six Divine Comedy-Jiao Hawthorn,cypress seed-Jujube kernel,tangerine peel-Pinellia pinellia,Shixiaosan-turmeric,Poria-Fushen,Atractylodes-Yam,Atractylodes-Trichosanthis Peel.Commonly used drug combination for tangerine turmeric-pinellia,yam-poria cocos-atractylodes,shixiaosan-suanzaoren-cypresskernel.Chest tightness,chest pain,back pain commonly used turmeric,laugh scattered;Before the heart area unwell person commonly used ginkgo leaf,tuckahoe,jujube kernel;Flustered shortness of breath with jujube kernel,cypress kernel.Tongue lilac commonly used trichosanthis peel,cypress seed,leech;Dark purple tongue commonly used three edges,mulberry leaves.Chinese yam and angelica are commonly used in qi deficiency syndrome.Yin deficiency syndrome commonly used jujube kernel,longan meat,licorice;Qi stagnation,blood stasis commonly used turmeric,smile scattered;Phlegm clip,commonly used stone calamus,trichosanthis peel.A total of 8 core recipes were obtained by cluster analysis,which were classified and sorted with self-formulated prescription for invigorating qi and activating blood circulation,Guipi decoction,Shenling Baizhu powder,Sijunzi He Erchen decoction,Tianma Gouteng Yin,Jiaotai pill and Suanzaoren decoction for addition and subtractions.Conclusion:Professor Fang Zhuyuan treated coronary heart disease angina pectoris with invigorating qi and activating blood circulation as the first method,associating with invigorating spleen and soothing liver,eliminating blood stasis and heat,reconciling qi and blood,etc.Yi Qi Huo Xue prescription Wenqing elimination and tonifying,Gan Wen can supplement,Xin San bitter catharsis,Gan Liang can moisten,so that tonifying qi without stagnation,removing blood stasis without injury,clearing heat without consuming yin.It provides valuable experience for the treatment of coronary heart disease with traditional Chinese medicine,and further enriches the treatment scheme and theoretical basis of coronary heart disease.
