Observation On The Clinical Curative Effect Of Nourishing Yin And Clearing Heat In The Treatment Of Diabetic Dry Eye With Yin Deficiency And Heat Exacerbation

Objective:This article uses the method of nourishing yin and clearing heat in combination with sodium hyaluronate eye drops to treat patients dry eyes in diabetic patients with yin deficiency and heat richness,compared to using sodium hyaluronate eye drops alone to observe the changes in symptoms and signs of patients before and after treatment,and evaluate the clinical efficacy of the prescriptions of nourishing yin and clearing heat.This provides some clinical evidences for the treatment of diabetic dry eye with yin deficiency and heat exacerbation by traditional Chinese medicine.Methods:A randomized controlled clinical trial design method was used to select 100 patients(200 eyes)of Yin deficiency and heat type 2 diabetes with dry eye who were treated in the Department of Ophthalmology and Endocrinology,Jiangsu Provincial Hospital of Traditional Chinese Medicine from August 2019 to August 2020.Group 50 cases(100 eyes),control group 50 cases(100 eyes),the control group was treated with sodium hyaluronate eye drops.On the basis of the control group,the traditional Chinese medicine group was orally taken Chinese medicine decoctions(Asparagus,Ophiopogon,Chuanhuanglian,Dendrobium,Bidens,Trichosanthes,Huai Yam,Chrysanthemum,Pueraria lobata,Prunella vulgaris),and added or subtracted according to the card.Observe and record the tear break-up time(BUT),corneal fluorescence staining(FL),Schirmer I test(SIT),fasting blood glucose(FBG),postprandial 2hours blood-glucose(2hPBG)and subjective symptom scores of the traditional Chinese medicine group and the control.group before and after 4 weeks of treatment.The SPSS26.0 statistical analysis software is used to calculate and analyze Clinical efficacy.Results:(1)There were no statistically significant differences in BUT,FL,SIT and symptom scores in diabetic patients with Yin-deficiency-heat type diabetes of different age,gender,course of disease,BMI,and fasting C peptide(P>0.05).(2)After treatment,the two groups of patients had prolonged BUT,increased SIT,decreased FL,and decreased symptom score.Compared with before treatment,the difference was statistically significant(P<0.05).The FBG and 2hPBG of the Chinese medicine group after treatment were lower than before treatment,and the difference was statistically significant(P<0.05).(3)Compared with the two groups after treatment,the traditional Chinese medicine group was better than the control group in increasing tear secretion and improving clinical symptoms,and the difference was statistically significant(P<0.01),but there was no statistical difference in BUT and FL(P>0.05).(4)In the two groups under the symptom curative effect evaluation standard,the effective rate of the traditional Chinese medicine group was 68%,and the effective rate of the control group was 32%.The difference was statistically significant(P<0.01).Under the disease and syndrome curative effect evaluation standard,the effective rate of the traditional Chinese medicine group 80%,the control group has an effective rate of 50%,and the difference is statistically significant(P<0.01).Conclusion:(1)The severity of dry eye in diabetic patients with yin deficiency and heat richness has nothing to do with their age,sex,course of disease,BMI,and fasting C-peptide.(2)The method of nourishing yin and clearing heat combined with sodium hyaluronate eye drops can significantly improve the clinical symptoms of dry eyes in T2DM patients with yin deficiency and heat richness.It has the effects of stabilizing the tear film,repairing corneal epithelium,and promoting tear secretion.(3)The effect of nourishing yin and clearing heat in diabetic dry eye patients with yin deficiency and heat richness is significantly better than that of the control group,especially in promoting tear secretion and improving systemic symptoms.The prescription is effective in nourishing yin and moisturizing dryness.It has good clinical significance and can be applied to patients with Yin deficiency diabetes.(4)The method of nourishing yin and clearing heat can appropriately control fasting and postprandial blood sugar,which is of great benefit to patients with diabetes.(5)In this experiment,there were no obvious adverse reactions in the method of nourishing yin and clearing away heat and the use of sodium hyaluronate eye drops,which has a certain degree of safety.
