| Objective:In recent years,the incidence of lumbago has gradually increased,second only to the disease of respiratory,but there are many patients with lumbago without obvious pathology and imaging evidence,which in traditional Chinese medicine can be attributed to blood-stasis lumbago,cold-dampness lumbago,kidney-deficiency lumbago and other types of lumbago.In the Song of Four General Acupoints,it is said that "Treating lumbar-back diseases by Weizhong point ",which means that for the diseases of the lumbar-back,the Weizhong point can be used for treatment,and most of the acupoints chosen by ancient and modern doctors for the treatment of lumbago include the Weizhong point,but for the treatment of blood-stasis lumbago,cold-dampness lumbago and kidney-deficiency lumbago,there is no detailed discussion in ancient literature and modern reports on whether the bleeding therapy of Weizhong point is all applicable and which type of lumbago is more applicable.In this study,the patients with lumbago were studied by observing the clinical effect of bleeding at Weizhong point on the treatment of blood-stasis lumbago,cold-dampness lumbago and kidney-deficiency lumbago,to explore the adaptive range and the best indication of "Treating lumbar-back diseases by Weizhong point".Methods:According to the objective of the study and the standard of the subjects,90 cases of blood-stasis lumbago,cold-dampness lumbago and kidney-deficiency lumbago were selected,30 cases in each group,and 90 cases were all treated by bleeding at Weizhong point.The subjects’ diagnostic criteria were drawn up in accordance with the Criteria for theDiagnosis and Treatment of TCM Diseases developed by the State Administration of Traditional Chinese Medicine,the types of blood-stasis lumbago,cold-dampness lumbago and kidney-deficiency lumbago were formulated according to Acupuncture and Moxibustion Therapeutics published by China Traditional Medicine Press,the diagnostic criteria of Western medicine are based on the 2007 American Society of Physicians and pain clinical practice guidelines for chronic nonspecific low back pain(CNLBP).The specific treatment is as follows:Three groups of patients were selected for blood-letting treatment of bilateral Weizhong points,one side of Weizhong point after blood-letting treatment of the other side Weizhong point.Using the needle of disposable medical syringe no.6.5,ask the patient to take a standing position.The doctor ligated the upper end of Weizhong point(near the end of the heart)with a rubber tube and gently patted the popliteal fossa to expose the blue and purple veins.After wearing sterile gloves,disinfecting the skin of Weizhong point with iodophor three times and then removing iodine with 75% alcohol,the doctor pressed the lower end of the patient’s Weizhong point with the left thumb and held the needle of a disposable medical syringe 6.5 in his right hand,aiming at the blue and purple veins of Weizhong point,remove the needle immediately after 2-3mm insertion,loosen the rubber tube tied at the upper end and let the blood flow out naturally.After the blood flow stops by itself,cover the pinhole with a medical infusion stick to avoid infection.Instruct the patient to keep the blood letting part of the puncture clean,do not take a bath within 48 hours as far as possible in order to avoid infection of the pinhole and bring adverse consequences.All the 90 patients were evaluated with VAS and PPI score.The therapeutic evaluation criteria are based on the Pain Science published by Liaoning Traditional Chinese Medicine Press.The statistical software uses SPSS25.0.The research contents are as follows:(1)Gender,age,course ofdisease,pre-treatment VAS and PPI scores of the three groups were counted,and the comparability of the three groups was analyzed;(2)After treatment,the VAS and PPI scores of three groups were compared before and after treatment,and the adaptive range of bleeding at Weizhong point were analyzed;(3)The differences of VAS and PPI scores before and after treatment were compared to analyze the best indications of bleeding at Weizhong point.According to the data,the clinical evaluation of the treatment results of the three types of syndrome patients was carried out objectively,and the adaptive range and the best indication type of bleeding in the Weizhong point were selected.Result:1.After statistical analysis,there was no significant difference in gender,age,course of disease,VAS score and PPI score before treatment among the three groups(P>0.05).2.Before and after treatment,there were significant differences in VAS and PPI scores between blood-stasis lumbago group and cold-dampness lumbago group(P<0.01,P<0.01),however,there was no significant difference in VAS and PPI before and after treatment(P>0.05)in the Group of kidney-deficiency lumbago,which showed that bleeding was suitable for blood-stasis lumbago and cold-dampness lumbago,but not for kidney-deficiency lumbago.3.Before and after treatment,there were significant differences in VAS and PPI scores between the two groups(P<0.01,P<0.01),the results showed that the analgesic effect of the blood-stasis lumbago group was better than that of the cold-dampness lumbago group and the kidney-deficiency lumbago group.There were significant differences in VAS and PPI scores between the cold-dampness lumbago group and the kidney-deficiency lumbago group before and after treatment(P<0.01),it is suggested that the analgesic effect of cold-dampness lumbago groupis better than that of kidney-deficiency lumbago group.4.The total effective rate was 96.67% in the blood-stasis lumbago group,66.67% in the cold-dampness lumbago group,10.00% in the kidney-deficiency lumbago group.The total effective rate in the blood-stasis lumbago group was higher than that in the cold-dampness lumbago group,conclusion the clinical effect of blood-stasis lumbago group is better than that of cold-dampness lumbago group,and that of cold-dampness lumbago group is better than that of kidney-deficiency lumbago group.Conclusion: The results of this study show that bleeding at Weizhong point is suitable for blood-stasis lumbago and cold-dampness lumbago,but not for kidney-deficiency lumbago,and it also provides a scientific basis for the adaptive range and the best indication of "Treating lumbar-back diseases by weizhong point". |
