Objective: To explore the effect of autologous platelet-rich plasma on IL-1,IL-6 and TNF-α in synovial fluid of TMJOA in rabbit,and to provide a theoretical evidence for clinical treatment of TMJOA.Methods: A total of 36 New Zealand white rabbits were used in the present study.All rabbits were divided into four groups,including blank control group,model control group,sodium hyaluronate group and PRP group(n = 9).Type II collagenase was injected into right TMJ cavity for one time to establish osteoarthritis in model control group,sodium hyaluronate group and PRP group.Equivalent isotonic Na Cl solution was injected into the right TMJ of the blank group.PRP was injected two times(once a week)into the right TMJ in the PRP group 2 weeks after model established.Equivalent isotonic Na Cl solution was injected into the right TMJ in the control groups and model control group,and equivalent sodium hyaluronate was injected into the right TMJ in the sodium hyaluronate group.Three rabbits in each group were used to extract synovial fluid of right TMJ at 2 weeks after modeling,2 weeks and 4 weeks after injection,and detect the expression level of IL-1,IL-6,TNF-α in the synovial fluid.Sacrificed animals and resected the right condyle after extracted synovial fluid at the same period,general observation was used to observed the situation of obtained samples,and the HE staining sections of condyle was made for histopathologicalobservation.Results: 1.General specimen: The condyle surface of the blank control group was smooth and continuous,and there were no obvious depressions or deletions at 2 weeks after modeling.The other three groups showed typical OA disease-like changes such as condylar cartilage defects.There was no significant change in the blank group at the 2,4 weeks after injection.Cartilage remodeling occurred in the model control group,sodium hyaluronate group and PRP group,and the cartilage surface still had depressions and deletions,while the model control group showed osteophytes atthe 4 weekafter injection.2.HE stained : The condyle cartilage was intact in the blank group at 2 weeks after modeling,the other three groups showed signs of destruction of the condyle cartilage at the same period,such as fracture of the cartilage layer or partial loss of the cartilage layer;There was no significant change in the blank group at 2,4weeks after injection.The cartilage appeared moderately cracked,and the layer was disordered in the model control group,PRP group,and sodium hyaluronate group at 2 weeks after injection;The layer of cartilage was unclear,proliferated,thickened and osteophytes formed in the model control group at 4weeks after injection,The cartilage in the PRP group and the sodium hyaluronate group became thicker with unclear layers,and damaged cartilagesurfacelayers were visible at4 weeks afterinjection.In all periods,the Neo’s condylar cartilage score of the blank control group was lower than that of the other groups(P<0.05),There was no significant difference in the score of condylar cartilage among the other three groups(P>0.05).3.The expression of IL-1,IL-6,and TNF-α in synovial fluid: The expression of IL-1,IL-6and TNF-α in the synovial fluid of the model control group,sodium hyaluronate group and PRP group were higher than the blank group(P<0.05),but there were no difference among the group at2 weeks aftermodeling(P>0.05).The expression of IL-1,IL-6 and TNF-α in the the model control group was higher than the blank control group at the same period at 2,4 weeks after injection(P>0.05),the expression of IL-1,IL-6 and TNF-α in the sodium hyaluronate group were lower than the model control group(P<0.05),and higher than the blank control group at the same period(P<0.05);the expression of IL-1 and IL-6 in the PRP group was lower than the model control group(P<0.05),and higher than the blank control group at 2,4 weeks after injection(P<0.05),the expression of TNF-α in the PRP group was lower than that of the model control group(P<0.05),and there was no significant differencefrom theblank control group at thesame period(P>0.05).There was no significant difference in the expression of IL-1 and IL-6 between PRP group and sodium hyaluronate group at 2 weeks after injection(P > 0.05),The expression of TNF-α of PRP group was lower than that in sodium hyaluronate group at the same period;there was no difference in IL-6 expression between PRP group and sodium hyaluronate group at 4 weeks after injection,but the expression of IL-1 and TNF-α of PRP group was lower than sodium hyaluronate group at same period.Conclusion: PRP injections could reduce the expression of IL-1,IL-6 and TNF-α in the synovial fluid of TMJ osteoarthritis,and the effect of inhibiting inflammation was better than sodium hyaluronate in TMJ osteoarthritis,but it had no significant effect on early cartilage destruction.
