| Objective: To explore the relationship between MTHFR gene C677 T polymorphism,homocysteine and blood lipid in patients with hypertension in northern Guangxi,and to provide guidance for the prevention and individualized treatment of hypertension in this area.Methods: A total of 339 hypertensive patients were selected from the outpatient department of cardiology of the Affiliated Hospital of Guilin Medical University from January 2018 to October 2020.According to the presence or absence of hyperhomocysteinemia,236 patients with H type hypertension and 103 patients with non-H type hypertension were divided into three groups.According to different genotypes,136 cases were CC type,153 cases were CT type and 50 cases were TT type.The polymorphism of MTHFR gene C677 T was detected by PCR-RFLP method,plasma lipid indexes and homocysteine concentration were determined by biochemical analyzer,and the correlation between MTHFR gene C677 T,Hcy and blood lipid was analyzed by SPSS statistical software.Results: There were significant differences in sex and homocysteine between H type and non-H type hypertension groups(P <0.05).The frequencies of CC,CT and TT genotypes were 36.02%,46.61% and 17.37% in the H-type hypertension group,and 49.51%,41.75% and 8.74% in the non-H-type hypertension group.The frequencies of CT and TT genotypes in H type hypertension group were higher than those in non-H type hypertension group(P < 0.05).The levels of TC,TG,LDL-C,Apo B and LP(a)in non-H type hypertension group were lower than those in H type hypertension group,and the levels of HDL-C and Apo AI were higher than those in H type hypertension group(P < 0.05).The levels of TG,TC,LDL-C,Apo B and LP(a)were positively correlated with the Hcy level,while the levels of HDL-C and Apo AI were negatively correlated with the Hcy level(P < 0.05).Logistic regression analysis showed that MTHFR C677 T mutant TT was a risk factor for H type hypertension,and the mutant TT was still a risk factor for H type hypertension after adjustment for gender factors(P < 0.05).Gender was also an influential factor for H type hypertension,and the prevalence of H type hypertension in males was higher than that in females(P < 0.05).TG,TC,LDL-C,Apo B,LP(A)concentrations TT >CC,HDL-C,Apo AI concentrations CC > TT in different genotype groups(P < 0.05).Conclusion: In northern Guangxi,compared with non-H type hypertension patients,the levels of HDL-C and Apo AI decreased more significantly in H type hypertension patients,and the levels of TG,TC,LDL-C,Apo B,LP(a)increased significantly.Mutant TT type in MTHFR gene C677 T may be a genetic factor in patients with H type hypertension,with a higher incidence in males.Hypertensive patients with TT type C677 T mutation of MTHFR gene have higher levels of TG,TC,LDL-C,Apo B,LP(a),and lower levels of HDL-C and Apo AI. |
