The Significance Of Detection Of Peripheral Serum IGF-1 And IGFBP-3 In The Clinical Diagnosis Of Patients With Primary Liver Cancer

Object:To explore the expression changes of peripheral serum IGF-1 and IGFBP-3 in patients with primary liver cancer,patients with liver cirrhosis,and healthy people,in order to explore the significance of early clinical diagnosis of primary liver cancer.Methods:Collected patients with primary liver cancer(group A),patients with liver cirrhosis(group B)and healthy adults(group A)diagnosed in Xinjiang Production and Construction Corps Hospital and the First Affiliated Hospital of Xinjiang Medical University from December 2016 to December 2019.Group C).Measure and record the levels of serum insulin-like growth factor-1(IGF-1),serum insulin-like growth factor binding protein-3(IGFBP-3),and alpha-fetoprotein(AFP)between each group.Data and serological data were analyzed statistically.Results: A total of 105 study subjects were collected,including 35 in group A,24 males and 11 females,with an average age of(55.14±10.18)years,AST of(65.88±16.83)U/L,and ALT of(61.01±13.70)U /L;There are 35 cases in group B,including 20 males and 15 females,with an average age of(60.94±10.79)years,AST of(38.61±14.60)U/L,and ALT of(44.84±11.69)U/L;There were 35 cases in group C,including 22 males and 13 females.The average age was(56.66±11.24)years old,AST was(25.25±10.61)U/L,and ALT was(27.13±8.04)U/L.There was no statistical difference in age and gender between the groups(P>0.05),and there were statistical differences in AST and ALT(P<0.05).The analysis showed that:(1)The serum IGF-1 levels of the three groups showed a skewed distribution,and the serum IGF-1 levels of the C group were higher than those of the A and B groups;(2)the serum IGFBP-3 levels of the A group were positive The level of serum IGFBP-3 in B and C showed a skewed distribution.The level of serum IGFBP-3 in group C was higher than that in groups A and B;(3)the median concentration of IGF-1 in groups A,B and C The numbers are 77.8(27.2,105.0),34.5(77.9,112.4),64.4(139.2,203.6);the median concentrations of IGFBP-3 in the three groups A,B,and C are 1.3(1.8,3.1),1.6(3.8,5.4),3.8(5.0,8.8),the differences in the concentrations of IGF-1 and IGFBP-3 between groups were statistically significant(P<0.05);(4)Serum IGF-1 and IGFBP-3 are in liver cancer The sensitivity is 88.6%,97.1%,and the specificity is 77.1%,82.9%.The area under the ROC curve is 0.895 and 0.880 respectively.The Yorden index is 0.657 and 0.800 respectively.(5)Correlation analysis between IGF-1 and AFP among the groups showed that the correlation coefficients were r=-0.047,p=0.790,r=-0.155,p=0.373,r=-0.166,p=0.341;IGFBP-3 and The correlation analysis of AFP was r=0.014,p=0.938,r=--0.292,p=0.089,r=-0.288,p=0.093.There was no correlation between serum IGF-1,IGFBP-3 and AFP in the three groups of subjects(P>0.05).Conclusion:(1)The concentrations of IGF-1 and IGFBP-3 in the peripheral serum of patients with primary liver cancer are lower than those of patients with liver cirrhosis and healthy people.There is a difference between liver cirrhosis and primary liver cancer,which can progress to liver cancer as liver cirrhosis and One of the research indicators for early diagnosis of liver cancer.(2)There is no correlation between peripheral serum IGF-1,IGFBP and AFP levels,and IGF-1,IGFBP-3 combined with AFP detection may have certain significance.
