Circumcision Combined With Frenulum Lengthening In The Treatment Of Premature Ejaculation

Objective: To investigate the efficacy and safety of circumcision combined with frenulum extension in the treatment of premature ejaculation combined with prepuce hyperplasia,and to further explore the pathogenesis and treatment of premature ejaculation.Methods: A total of 60 patients with premature ejaculation complicated with prepuce hyperplasia were collected from the urological department of the Second Hospital of Hebei Medical University from January 2020 to October2020.The 60 included patients were divided into the experimental group and the control group according to the principle of complete randomization.All of them were given the score of Premature ejaculation diagnostic tool(PEDT).At the same time,each patient was surveyed for 3 IELTs,and the average of the three vaginal ejaculation periods was taken as the standard.Among them,the experimental group received circumcision combined with frenulum length.and the control group received simple circumcision.The changes of PEDT score and IELT in patients were followed up by outpatient or telephone 2 and3 months after surgery,and the treatment effect of Premature ejaculation(PE)was observed.Results: In terms of PEDT score,preoperative PEDT score of the control group was(14.44±2.28)points,and the PEDT score decreased to(10.85±1.99)points and(10.70±2.13)points respectively 2 months and 3 months after surgery,with significant statistical differences between the two postoperative PEDT scores and the preoperative PEDT scores(P <0.05).The PEDT scores of the experimental group before surgery were(15.00±1.96)points,and 2months after surgery and 3 months after surgery,the PEDT scores decreased to(9.48±1.53)points and(8.92±1.35)points,respectively.Similarly,the PEDT scores of the 2 times after surgery were significantly different from those before surgery(P<0.05).Data analysis between the two groups showed that the PEDT scores of the two postoperative times were also statistically significant(P<0.05).In terms of IELT,the IELT of the control group was(1.59±1.01)min before surgery,and increased to(2.52±1.24)min 2 months after surgery and(2.33±1.58)min 3 months after surgery,respectively.There were significant statistical differences in IELT 2 months after surgery and 3 months after surgery compared with that before surgery(P<0.05).Experimental preoperative IELT(1.66±0.89)min,after 2 months and 3 months posto-peratively IELT respectively increase to(3.88±1.08)min and(4.12±1.37)min,also according to the results after 2 months and 3 months after surgery with preoperative IELT IELT all have significant statistical difference(P<0.05),respectively after 2 months and 3 months postoperatively IELT compared between groups,the experimental group and the control group also has significant statistical difference(P<0.05).Conclusions: For patients with premature ejaculation combined with excessively long forespuce,circumcision combined with frenulum extension has better therapeutic effect and stable effect compared with circumcision alone for premature ejaculation.Meanwhile,this operation is simple and easy to perform,and suitable for promotion.
