| ObjectiveThe purpose of this study is to explore the risk factors of gastrointestinal dysfunction in children with sepsis from the clinical data easily available,and propose possible preven-tion measures.So as to guide clinical prevention of gastrointestinal dysfunction in children with sepsis and ultimately reduce the occurrence of MODS,so as to improve the prognosis of children with sepsis.MethodsFrom June 2016 to May 2018,110 cases of pediatric patients diagnosed with sepsis(including severe sepsis and septic shock)were strictly screened out from the pediatric intensive care unit hospitalized in LIAOCHENG PEOPLE’HOSPITAL,and then divided into GID group and non-gid group according to the presence or absence of gastrointestinal dysfunction.Applications were retrospectively analyzed and case-control method,collected in the patient’s age,gender,PCIS score,CRP and PCT,Lac,oxygenation index,a total of 24blood albumin levels and so on the clinical data as independent variables,gastrointestinal dysfunction or failure will occur as the dependent variable,the single factor analysis and multivariable Logistic regression analysis,to find sepsis merger risk factors of gastroin-testinal dysfunction.Results(1)A total of 40 cases(36.4%)of 110 children with sepsis were associated with gastrointestinal dysfunction.Among them,49 cases were boys,accounting for 44.5%,and61 cases were girls,accounting for 55.5%.There were 40 cases of GID group,accounting for 36.4%,and 70 cases of non-gid group,accounting for 63.6%.(2)A total of 25 children in the gastrointestinal dysfunction group were diagnosed with severe sepsis,accounting for 62.5%.There were 27 cases of severe sepsis in the non-gastrointestinal dysfunction group,accounting for 38.6%,with statistically significant difference between the two groups(P<0.05).(3)Univariate analysis revealed 8 variables,including PCT,PLT,PCIS score,blood lactic acid value,Pa CO2,albumin level,and accompanying shock,MODS,showing statis-tically significant differences between the GID group and the non-gid group(P<0.05).(4)Multivariate Logistic regression analysis showed that variables still related to gastrointestinal dysfunction in sepsis were PCIS score,albumin level and blood lactic acid value(P<0.05).Conclusions and aignificance1.Decreased PCIS score,decreased albumin level and increased blood lactate level are risk factors for gastrointestinal dysfunction in children with sepsis.2.Clinical attention and early intervention measures should be paid to children with sepsis with the above high-risk factors,which is of great significance for reducing the inci-dence of GID and improving the prognosis of children with sepsis. |
