At present,China is in the critical period of building a well-off society in an all-round way and starting the 13th five year plan.As an important part of the whole social security system,the basic medical insurance system must be continuously improved to meet the needs of different groups of society for medical assistance.China’s rural population accounts for nearly half of China’s total population,and the labor force of the right age in the rural population is the mainstay of this group,which mainly bears the income responsibility of the whole rural family.According to the survey report of migrant workers,the number of migrant workers in China has reached 290 million.Therefore,it is reasonable to take the rural labor force as the research object.The incidence of catastrophic health expenditure is mainly measured by the proportion of medical expenditure in income.If it is greater than or equal to or less than a certain threshold,it is considered that catastrophic expenditure has occurred or no catastrophic expenditure has occurred.It is feasible to use it as a measure of basic medical insurance.If the rural labor force has high medical expenditure,its low income is prone to catastrophic expenditure,leading to the phenomenon of "poverty caused by illness" and "returning to poverty due to illness".As the most basic medical security system,basic medical insurance needs to be perfected.Therefore,it is of great significance to study the effect level and mechanism of basic medical insurance on the medical and health expenditure of rural labor force Higher theoretical and practical significance.This article uses CFPS data from 2012 to 2018 for a total of four years to study this issue.It mainly includes six parts.The first part is the introduction,the second part is the development status and problems of my country’s basic medical insurance system,the third part is the theoretical analysis of the medical insurance system,the fourth part is the data description,and the fifth part is the empirical study.Analysis,the sixth part is conclusions and countermeasures.In this paper,combined with Grossman’s health needs theory,the influencing factors of catastrophic health expenditures are analyzed theoretically,and then a binary Probit model is established to analyze the data.The regression results show that basic medical insurance reduces the incidence of catastrophic health expenditures of rural laborers to a certain extent,but the reduction effect is weak.In order to verify the robustness of the results,the standard for measuring catastrophic health expenditures,ordinary least squares and instrumental variables are changed.The results were tested for robustness in three aspects,and the results were still significant,reflecting the robustness of the results.Later,the heterogeneity analysis was conducted from the three aspects of different ages,incomes and mobility distances.Finally,this article puts forward a series of countermeasures and suggestions based on the empirical results.This study puts forward suggestions from four aspects: the government,social security institutions,medical service institutions and the insured.The government needs to improve the precision of poverty alleviation by medical insurance;the basic medical insurance should be guaranteed;the medical service institutions should improve the public welfare and the insured should improve their health awareness.So as to further improve the medical insurance system,promote the development of medical insurance,and promote social development.
