| While working in F nursing home,the author observed that the care service of institutional elderly care for the elderly with cognitive disorder is mainly physical care,and little attention is paid to other needs of the elderly.Although the cognitive function of the elderly is impaired to varying degrees,their various needs as social people still exist.In addition,in the development of the disease,the cognitive elderly will have various mental and behavioral symptoms.This makes the cognitive elderly face the challenges brought by aging,but also the complex difficulties brought by disease.After screening,this study conducted a one-year intervention study on 18 cognitive elderly in F nursing home.F the staff in the nursing home set up a case management team to comprehensively evaluate the physiological,psychological and social support of the cognitive elderly with the help of MMSE scale,Bi scale and social function activity questionnaire,combined with the observation of nurses in daily care and through interviews with the cognitive elderly and their families.The assessment found that the cognitive elderly need more personalized life care than the general elderly.Although the cognitive function of the elderly is impaired,they still have the needs of psychological support and social participation.In addition,the elderly with cognitive disorder in institutional housing are faced with the dilemma of shortage of nursing resources,difficulty in obtaining positive psychological support and discrimination due to diseases.This study evaluated the cognitive elderly and formulated a case management plan.From the perspective of whole person health,from the four dimensions of physical care,psychological care,social support and spiritual care,through the integration of internal and external resources,and the coordination of professional staff to intervene in the elderly with cognitive disorders.After one-year intervention,through the statistical analysis of the front and rear data of the quality of life of the cognitive elderly,combined with the interview with the cognitive elderly and their families,it is concluded that case management has a significant effect on improving the quality of life of the cognitive elderly in institutions.In addition,the implementation of case management promotes the rational division of labor and close cooperation among professional staff within the organization,which helps to improve the standardization and specialization of staff within the organization.The originally scattered resources outside the organization gradually realize connection and integration.The implementation of case management has practical significance for promoting nursing specialization,building resource network and creating a cognitive disease friendly atmosphere.It has the conditions and is worthy of promotion. |
