Objective:Based on transcranial Doppler(TCD),this study analyzed intracranial blood flow velocity,pulsatility index and resistance index of phlegm and blood stasis obstructing collaterals syndrome in acute cerebral infarction,and explored the correlation between TCM syndrome elements and intracranial blood flow mechanics of phlegm and blood stasis obstructing collaterals syndrome in acute cerebral infarction.Methods:prospective cross-sectional study was used in this study.The inclusion criteria were acute mild and severe cerebral infarction patients with phlegm and blood stasis obstructing collaterals syndrome within 72 hours of onset,and met TOAST classification of large atherosclerotic cerebral infarction and OCSP classification of partial anterior circulation.The general data and head MRI were collected,and the TCM syndrome,clinical symptoms and signs were analyzed on the day of admission and 7 days after admission SPSS 25.0 software was used to analyze the correlation between TCD indexes and TCM syndrome elements.Results:? In this study,75 patients with acute cerebral infarction of phlegm and blood stasis syndrome were collected.There were 55 males(73.3%)and 20 females(26.7%);The oldest was 80 years old and the youngest was 37 years old,with an average age of 63.13 years±74 years old;There were 45 cases of hypertension(60%),23 cases of diabetes mellitus(30%),19 cases of hyperlipidemia(25.3%),and 15 cases of coronary heart disease(20%).41 cases(54.7%)had a history of smoking and 33 cases(44.0%)had a history of drinking.? On the day of entering the group,the TCM syndrome elements were mainly internal wind,internal fire,phlegm dampness,blood stasis and other common syndromes,while deficiency syndrome(yin deficiency and Qi deficiency)was less.After 7 days,the main TCM syndrome elements were phlegm dampness and blood stasis,while the scores of internal wind and internal fire decreased.?The NIHSS score of patients with phlegm and blood stasis obstructing collaterals in acute stage of cerebral infarction on the 7th day was significantly lower than that on the 7th day.?The correlation analysis of TCM syndrome elements and TCD related indicators in patients with acute cerebral infarction with phlegm and blood stasis syndrome showed that when the score of internal wind syndrome elements increased,the intracranial blood flow velocity increased and the resistance decreased;When the integral of internal fire syndrome elements increased,the velocity of intracranial blood flow slowed down and the resistance index increased;When the integral of phlegm dampness syndrome elements increased,the velocity of intracranial blood flow slowed down and the resistance index increased;When the integral of blood stasis syndrome elements increased,the velocity of intracranial blood flow increased,and the pulsatility index and resistance index decreased;When the integral of qi deficiency syndrome elements increased,the velocity of intracranial blood flow increased;When the integral of yin deficiency syndrome elements increased,the velocity of intracranial blood flow slowed down and the resistance index increased.?Compared with normal blood flow group,abnormal blood flow group had higher age,lower blood pressure(systolic pressure and diastolic pressure),more drinkers,more history of smoking and drinking,and more NIHSS scores.The diastolic blood pressure of the two groups was statistically significant(P<0.05),and the rest was not statistically significant.
