Research On The Medication Rules Of Post-stroke Shoulder-hand Syndrome By External Treatment Of Traditional Chinese Medicine Based On Data Mining

Purpose:In this study,we searched the clinical literature on the topical treatment of shoulder-hand syndrome(SHS)after stroke in four major Chinese databases(CNKI,WIP,Wan Fang,CBM)in the past 30 years,and used data mining technology to count the data involved in the literature.Based on this analysis,we will understand the research status of external use of traditional Chinese medicine for the treatment of SHS,and explore the application rules of external use of traditional Chinese medicine for the treatment of SHS,so as to provide a theoretical basis for the diagnosis and treatment of this disease by modern doctors.Methods: Advanced search was conducted with the subject terms "shoulder-hand syndrome","SHS","reflex sympathetic dystrophy","reflex sympathetic dystrophy",and "complex regional pain",published from November 1990 From 30 th to November 30 th,2020,search the clinical literature on SHS after stroke by topical Chinese medicine in the four major Chinese databases,and screen them one by one according to the inclusion and exclusion criteria,until the literature that meets the criteria is found.Extract the general information of each document(including the funding status,the publication period of the document,the source of the document,the name of the journal,the area where the document was published,and the type of intervention of traditional Chinese medicine),as well as the drugs in each document,the types of traditional Chinese medicines and their characteristics and flavors.After standardizing the data,create an Excel table,and use SPSS 26.0 and IBM SPSS Modeler 18.0 software to perform frequency analysis,association analysis,and cluster analysis on the standardized data.Results:1.Literature search results: According to the inclusion and ranking criteria,111 documents that meet the standards were screened out.2.Literature general information results: the types of included literature include clinical efficacy observations,clinical case analysis,etc.,26 papers are funded,and the number of publications is mainly concentrated in the past 10 years.The main source of the literature is journal papers.Most areas are Guangdong,Zhejiang,Jiangsu,and Henan.The most frequently used traditional Chinese medicine intervention is traditional Chinese medicine fumigation.3.Results of general medication rules for external use of traditional Chinese medicine in the treatment of SHS:(1)There are 143 kinds of Chinese medicines in 111 literatures,and the total frequency of use is 1086 times.There are 34 traditional Chinese medicines with a frequency of ?9times,including safflower,chinensis,cassia,chuanxiong,etc.,with a cumulative frequency of 824 times,accounting for 75.87% of the total frequency.They are high-frequency traditional Chinese medicines for the treatment of SHS.(2)According to the classification of the meridian of the nature and flavor of the medicine,the common medicinal properties are the mild drugs,the pungent medicine is commonly used in the five flavors,and the drugs that enter the liver meridian are commonly used in the meridian.In terms of drug classification,the most frequent is the anti-rheumatic drugs,followed by the blood circulation and blood stasis drugs,and the second is the anti-epidermic and tonic drugs.(3)High-frequency Chinese medicines can be grouped into 5 categories through cluster analysis: the first category: safflower,glutinous grass,Chuanwu,mulberry twig,grass black,Qianghuo,Liaohuo,Asarum,Gentiana.The second category: Osmanthus fragrans,Toucanthus,Caulis spatholobi,Weilingxian,Mugwort,Turmeric,Parsnips,Passepartout.The third category: Angelica,Chuanxiong,Dilong,Astragalus,Red peony root,Peach kernel,Licorice,Scorpion,Achyranthes bidentata.The fourth category:Frankincense,Myrrh,Angelica dahurica,Corydalis,Salvia miltiorrhiza.The fifth category: Evodia,mustard seeds.(4)The high-frequency Chinese medicines were analyzed for correlation,and the correlation analysis showed that there were 16 groups of association laws of two kinds of medicines,12 groups of association laws of three kinds of medicines,and 3 groups of association laws of four kinds of medicines.Commonly used two-medicine compatibility combinations are: Chuanwu + safflower,mulberry twig + safflower,Caowu + safflower,Caowu + Chuanwu,frankincense + myrrh and so on.Commonly used combinations of three medicines are: Chuanwu + safflower + grass black,Ligusticum chuanxiong + angelica + safflower,Chuanwu + safflower + safflower,Chuanwu + sclerophylla + saccharum,Angelica + safflower + red Flowers,sclerophyllum sylvestris + safflower + safflower,Tougucao + safflower + safflower,cassia twig + safflower + safflower,Chuanxiong + safflower + safflower.The commonly used combination of four herbs is: grass black + chuan black + safflower +safflower.4.The results of the prescription of traditional Chinese medicine fumigation in the treatment of SHS:(1)Among the 111 articles included,the most frequently used traditional Chinese medicine intervention is traditional Chinese medicine fumigation,with a total frequency of 36 occurrences.(2)36 literatures involved 74 Chinese medicines.The total frequency of all Chinese medicines was 374 times.There were 16 medicines with a frequency of ?9 times.They were safflower,radix sinensis,angelica,cassia twig,etc.,and 16 Chinese medicines.The total frequency of use is 233 times,accounting for 62.30% of the total frequency of Chinese medicine.It is a high-frequency Chinese medicine.(3)Correlation analysis of 16 traditional Chinese medicines with a frequency of ?9times showed that the commonly used combinations of two medicines are: myrrh +frankincense,safflower + safflower,chuanwu + grass black,radix sinensis + Qianghuo,and Shen Muscle grass + grass black,safflower + grass black,glutinous grass + Wei Lingxian.Conclusion:1.In recent years,the quality of literature research and the level of academic research on the topical treatment of SHS with traditional Chinese medicine have gradually improved.The number of publications in the southern region is more than that in the northern region.The more commonly used traditional Chinese medicine intervention is traditional Chinese medicine fumigation.2.The core medicines for external use of traditional Chinese medicine for the treatment of SHS are 34 traditional Chinese medicines such as safflower,radix sinensis,cassia twig,and chuanxiong;the main properties are warm medicines,the main medicinal tastes are pungent medicines,and the main meridian of the medicines is to enter the liver meridian;In the classification of drugs,the use of rheumatism drugs is the most frequently used,followed by activating blood and removing blood stasis drugs,followed by anti-surgical drugs and tonic drugs;after association analysis and cluster analysis,the drug combination is obtained,and the principle of its formulation is analyzed.The findings are consistent with the lack of righteousness of the disease,and the etiology and pathogenesis of "wind,cold,dampness,and stasis" are mixed.3.The core medicines of traditional Chinese medicine fumigation treatment of SHS are safflower,radix sinensis,angelica,cassia twig and other 16 traditional Chinese medicines;commonly used medicine pairs are myrrh + frankincense,safflower + duhuo,chuanwu + grass black and so on.
