Objective:Quantitative flow ratio is a new measure of coronary physiology assessment and has a good correlation with fractional flow reserve.The purpose of this study was to explore the correlation between the sum of quantitative flow ratio(3V-QFR)in patients with stable three-vessels coronary artery disease.Methods:The study has included 110 patients in our hospital with stable three-vessels coronary artery disease diagnosed by coronary angiography from June 2021 to September2021,including 87 males and 23 females,with an average age of 65.40±9.14 years old,all of them had complete clinical data.After completing coronary angiography,we selected two coronary angiography images with a projection angle > 25°,and used the second-generation quantitative flow ratio detector(Angio Plus 2.0(V2.0.2.0),Shanghai Pulse Medical Imaging Co.,Ltd.)to analyze quantitative flow ratio of LAD,LCX and RCA,then calculated 3V-QFR.According to the median of 3V-QFR,the patients were divided into low score group(3v-QFR<2.17,55 cases)and high score group(3v-QFR≥2.17,55 cases).Two independent samples t-test or Mann-Whitney U test was used to compare the measurement data between groups;for enumeration data,the chi-square test was used to compare difference between groups.Correlations between clinical data and3V-QFR were compared using Spearman correlation analysis.Logistic stepwise regression was used to analyze the related factors affecting the grouping of 3V-QFR.The selected influencing factors were further analyzed by receiver operating characteristic analysis and the area under the curve was used to evaluate their effectiveness in predicting the grouping of 3V-QFR.P<0.05 was considered statistical significant,and P<0.01 was considered obviously statistical significant.Results:Compared with the high score group,the low score group had lower high-density lipoprotein cholesterol(1.11mmol/L vs 1.31mmol/L,P<0.05),while the low-density lipoprotein cholesterol was higher(2.56(2.22,2.97)mmol/L vs 2.31(1.94,3.03)mmol/L,P<0.05),the estimated glomerular filtration rate was lower(94.68±23.01 m L/min vs104.13±23.78 m L/min,P<0.05),and the number of lesions was higher(4(3,5)vs 3(3,4),P<0.01),and a higher functional SYNTAX score(17.00(12.00,23.00)vs 9.00(0.00,17.00),P<0.01).The correlation analysis between the clinical data and3V-QFR between two groups showed that low-density lipoprotein cholesterol,serum creatinine,Canadian Society of Cardiovascular Disease angina pectoris class,New York cardiac function class,SYNTAX score and 3V-QFR had a weak negative correlation(-0.3<r<0,P<0.05),functional SYNTAX score was strongly negatively correlated with 3V-QFR(-1.0<r<-0.5,P<0.01),high-density lipoprotein cholesterol was weak positive correlated with 3V-QFR(0<r<0.3,P<0.05),and the number of lesions had a moderate positive correlation with 3V-QFR(0.3<r<0.5,P<0.05).Logistic regression analysis showed that low-density lipoprotein cholesterol,serum creatinine,left ventricular ejection fraction,SYNTAX score,and functional SYNTAX score were related to 3V-QFR(P<0.05).Receiver operating characteristic curve analysis showed that low-density lipoprotein cholesterol and functional SYNTAX scores had predictive value for 3V-QFR(AUC were 0.68 and 0.74 respectively,P<0.05),which was the independent predictors of 3V-QFR.Conclusion:Low-density lipoprotein cholesterol and functional SYNTAX score were significantly correlated with 3V-QFR in patients with stable three-vessels coronary artery disease,and both of them were independent predictors of 3V-QFR.
