Objective :In this paper,the concentration of tumor necrosis factor-α(TNF-α)in gingival crevicular fluid of patients with chronic periodontitis(CP),chronic obstructive pneumonia(COPD)and CP combined with COPD was detected,so as to analyze the expression level of TNF-α in gingival crevicular fluid and the correlation between TNF-α and various periodontal indexes and lung function indexes,and to explore the expression level and changes of TNF-α in gingival crevicular fluid of patients with CP,COPD and CP combined with COPD.To determine whether TNF-α can be used as an indicator of disease detection,so as to provide new prevention and treatment ideas for CP and COPD.Research Methods :1.Subjects : From November 2021 to September 2022,patients who were admitted to the Department of Stomatology and the Department of Respiratory and Critical Care Medicine of Liaoning Provincial People ’s Hospital were screened and divided into 4groups according to clinical diagnosis,21 patients with chronic periodontitis(CP group),19 patients with chronic obstructive pneumonia(COPD group),21 patients with chronic periodontitis and chronic obstructive pneumonia(CP + COPD group),and 22 healthy people who were examined in the physical examination center during the same period were selected as the control group(H group),a total of 83 cases.2.Data collection and examination analysis : The general conditions of all subjects were examined and recorded by group,including age,sex,BMI,daily brushing times and intraoral tooth loss.The periodontal conditions of each group were examined and recorded,including probing depth(PD),attachment loss(AL),plaque index(PLI),sulcus bleeding index(BI)and the number of missing teeth(M).The lung function indexes of each group were recorded,including Vc MAX(maximum vital capacity),FVC(forced vital capacity)and FEV1 / FVC(ratio of forced expiratory volume in the first second to forced vital capacity).The gingival crevicular fluid of four groups of subjects was collected by moisture absorption paper tip,and the concentration of TNF-αin gingival crevicular fluid samples of each group was measured by enzyme-linked immunosorbent assay(ELISA).Statistical analysis : SPSS 27.0 software was used to analyze all data between groups and within groups.Research results1.Analysis and comparison of periodontal indexes : The periodontal indexes of CP group and CP + COPD group were higher than those of COPD group and H group(P <0.05);the periodontal indexes of CP + COPD group were higher than those of CP group(P < 0.05).2.Comparison of lung function related indicators : Vc MAX,FVC and FEV1 / FVC indicators in the CP + COPD group were significantly lower than those in the other three groups(P < 0.05).3.Comparison of the expression level of TNF-α in gingival crevicular fluid : The expression level of TNF-α in CP + COPD group was the highest(P < 0.05),and the expression level of TNF-α in CP group and COPD group was higher than that in H group(P < 0.05).4.Correlation analysis between the expression level of TNF-α and the amount of gingival crevicular fluid in the four groups of gingival crevicular fluid samples : The expression level of TNF-α was positively correlated with the amount of gingival crevicular fluid(r = 0.097,p < 0.01).5.Correlation analysis between the expression level of TNF-α in gingival crevicular fluid and periodontal indexes : The expression level of TNF-α in gingival crevicular fluid of the four groups was significantly positively correlated with AL,PD,BI,PLI and M(p < 0.01).6.Correlation analysis between the expression level of TNF-α in gingival crevicular fluid and various lung function indexes : The expression level of TNF-α in gingival crevicular fluid of the four groups was significantly negatively correlated with Vc Max,FVC and FEV1 / FVC(p < 0.01).Conclusion :1.The levels of periodontal indexes and pulmonary function indexes in patients with periodontitis complicated with chronic obstructive pneumonia are more serious than those in patients with simple periodontitis and simple chronic obstructive pneumonia,suggesting that the two diseases affect and promote each other.2.The expression level of TNF-α detected in gingival crevicular fluid samples of periodontitis combined with chronic obstructive pneumonia group was higher than that of simple periodontitis and simple COPD patients,and the content was negatively correlated with lung function indexes and positively correlated with periodontitis indexes,indicating that the expression level of tumor necrosis factor-α was closely related to the occurrence and progression of chronic periodontitis and chronic obstructive pneumonia,suggesting that TNF-α can be used as a clinical detection and diagnostic index of CP and COPD. |
