The Sensitivity Difference Between The Glans Penis And Penile Shaft In Primary Premature Ejaculation

Objective:The penis serves as an important sexual receptor organ during sexual intercourse,which transmits signals of perceived stimulation to the relevant centers and subsequently produces responses such as erection and ejaculation.The penis consists of the glans penis and the corpus penis,which are very different both histologically and in terms of innervation.Could this difference lead to different stimulation signals being received by the relevant areas within the brain? Therefore,we conducted this study to explore whether there is a difference in sensitivity when the glans penis or the penile shaft used as a receptor.And penile hypersensitivity is the whole penis hypersensitive or part of the penis hypersensitive?Methods:This study collected the thresholds,latencies and wave amplitudes of somatosensory evoked potentials(SSEPs)from July 2017 to March 2021 in patients with primary premature ejaculation at the First Affiliated Hospital of Dalian Medical University.After screening by inclusion and exclusion criteria,a total of 290 patients with primary premature ejaculation were included,and their neurophysiological data(biosensory thresholds,latencies,and wave amplitudes)were collected.The normality of the continuous variables was first tested by K-S test.The latency and biosensory thresholds of both the glans penis and penile corpora conformed to a normal distribution,and the Student’s t test was used to compare the differences between them.The wave amplitudes of the glans penis and penile corpora did not conform to a normal distribution,and the Mann-Whitney U test was used to compare the variability between them.Correlations between the biosensory thresholds,latencies,and wave amplitudes of the penile glans and penile body were compared by Pearson’s correlation or Spearman’s correlation.Consistency of hypersensitivity between penile glans and penile corpora was assessed by Mc Nemar ’s test.p<0.05 was considered a statistically significant difference.Results:SSEPs thresholds,latency and wave amplitude were significantly different between the glans penis and penile corpora in patients with primary premature ejaculation(P<0.05).Biosensory thresholds were significantly higher in the penile corpora than in the penile glans(p = 0.0000),latency was significantly longer in the penile glans than in the penile corpora(p = 0.0000),and wave amplitude was significantly lower in the penile glans than in the penile corpora(p = 0.0000);correlations existed between the penile glans and penile corpora in patients with premature ejaculation:(a)there was a weak correlation between the biosensory thresholds of the penile glans and penile corpora(r = 0.3795,p<0.001).(b)There was a significant correlation between the latency of the glans penis and the latency of the penis body(r= 0.6198,p<0.001).(c)There was a significant correlation between the wave amplitude of the glans penis and the body of the penis(r=0.5439,p < 0.001).The difference in latency between the middle of the penile body and the corpus penis was compared and the results were not statistically significant.A total of 141(48.62%)premature ejaculation patients in this study were hypersensitive to either the glans penis or penile body,of which 50 patients(35.5%)were hypersensitive to both the glans penis and penile body,14 patients(9.93%)were hypersensitive to the glans penis only,and 77 patients(54.6%)were hypersensitive to the penile body only.There was a significant difference in hypersensitivity between the glans penis and the corpora penis(P < 0.001).Conclusion:In this study,we found that the stimulation signals perceived through the glans penis and the corpora penis are inconsistent in patients with primary premature ejaculation.Penile hypersensitivity does not necessarily mean that the entire penis is hypersensitive;it may be either single penile glans hypersensitivity or single penile body hypersensitivity.We classified penile hypersensitivity in patients with primary premature ejaculation into three categories: mono-penile glans hypersensitivity,mono-penile body hypersensitivity and all penile hypersensitivity,and proposed a new concept of penile hypersensitive area and penile effective sensory area.We believe that ejaculatory dysfunction can be improved by modulating the matching of penile effective receptive zone and penile hypersensitivity zone,which may provide new ideas for future research on the diagnosis and treatment of primary ejaculatory disorders.
