Objective:According to the epidemiological investigation and gastroscopy data of areas with high incidence of gastric cancer in Wuwei City,to explore the risk factors of chronic atrogastritis(CAG),so as to provide a scientific basis for the prevention and treatment of CAG.Method:In Wuwei City,Gansu Province,an area with high incidence of gastric cancer,epidemiological questionnaire survey and gastroscopy screening were conducted.The patients diagnosed with CAG through gastroscopy were the case group.The control group was selected from the group diagnosed with chronic non-atrophicgastritis(CNAG)and normal gastric mucosa during the same period.The independent risk factors of CAG were determined by univariate and Iogistic regression analysis,including social demographic characteristics,diet,living habits,Helicobacter pylori(Hp)infection and previous disease history.Results:1.The prevalence rate of CAG in the general population was 74.4%.Gender,age,24.00 < Body mass index(BMI)≤28.00,consumption time of vegetables and fruits from January to April,preference for fast eating,previous history of peptic ulcer and Hp infection were independent risk factors for CAG.2.The infection rate of Hp in the total population was 53.0%.The prevalence of CAG in Hp-positive population was 77.5%.Gender,age,vegetable and fruit consumption time from January to April,and fast eating were independent risk factors for CAG in HP-positive population.The prevalence of CAG in HP-negative population was 70.8%.Gender,age,BMI≤18.50,24.00 < BMI≤28.00,vegetable and fruit consumption time from January to April,fast eating preference were independent risk factors for CAG in Hp-negative population.3.The prevalence rate of CAG in male population was 78.4%.The prevalence rate of CAG in men aged ≤40 years was 58.8%.The consumption time of vegetables and fruits from January to April each year,Hp infection was an independent risk factor for CAG in men aged ≤40 years.The prevalence rate of CAG was 73.8% in men aged less than 40 years or less than 50 years old.Family income of 10,000 to 20,000 yuan,fast eating and Hp infection were independent risk factors for CAG in men aged less than40 years or less than 50 years old.The prevalence rate of CAG was 84.7% in men aged50 years or less than 60 years old.Family income of 10,000 to 20,000 yuan,fast eating and Hp infection were independent risk factors for CAG in men aged 50 years or less than 60 years old.The prevalence of CAG in men aged > 60 years was 90.0%,and Hp infection was an independent risk factor for CAG in men aged > 60 years.4.The prevalence rate of CAG in female population was 70.8%.The prevalence rate of CAG in women aged ≤40 years was 54.4%,and Hp infection was an independent risk factor for CAG in women aged ≤40 years.The prevalence rate of CAG was 66.1%in women aged 40 years or less than 50 years old.24.00 < BMI≤28.00,fast eating and Hp infection were independent risk factors for CAG in women aged 40 years or less than 50 years old.The prevalence rate of CAG was 77.7% in women < 50 years old ≤60years old,24.00 < BMI≤28.00,BMI > 28.00,eating time of vegetables and fruits from April to June,Hp infection was an independent risk factor for CAG in women < 50 years old ≤60 years old.The prevalence rate of CAG in women > 60 years old was85.9%,and BMI > 28.00 was an independent risk factor for CAG in women > 60 years old.Conclusion:1.The prevalence rate of CAG was high in the total study population,and age,gender,BMI,eating time of vegetables and fruits,fast eating and Hp infection may be the main risk factors for CAG.2.The infection rate of Hp was relatively high in the total study population.Gender,age,eating time of fruits and vegetables and fast eating may be the main risk factors for CAG in Hp-positive population.In the absence of Hp infection,age,sex,vegetable and fruit consumption time,BMI,fast eating may be the main risk factors for CAG.3.Compared with the female group,the prevalence of CAG in male group is higher,and the prevalence of CAG in both male and female groups increases with the increase of age.Hp infection is related to the risk of CAG in each age group,and Hp infection may increase the risk of CAG with age.Fast eating,time of vegetable and fruit consumption and Hp infection may be common risk factors for CAG in male and female groups.Family income may be a risk factor unique to male CAG;BMI may be a specific risk factor for CAG in women.
