| ObjectiveBased on the large-scale natural population cohort of gastric cancer established in Wuwei City,an area with a high incidence of upper digestive tract tumors,the epidemiological investigation data,laboratory examination,and H.pylori screening and other baseline data of the patients clearly diagnosed as gastric polyps during gastroscopy screening were studied and analyzed to explore the risk factors of gastric polyps in this area,providing clinical basis for the prevention and treatment of gastric polyps.MethodsAccording to a large-scale natural population cohort established in Wuwei City,Gansu Province,patients diagnosed with gastric polyps by gastroscopy were selected as the case group,and patients diagnosed with chronic non-atrophic gastritis by gastroscopy were included in the control group.Baseline data and related clinical physical examination data of the included subjects were analyzed to explore the risk factors related to gastric polyps.Variables involved in this study included general status,diet and lifestyle,medical history,anthropometric indicators,and clinical laboratory test indicators.Firstly,the position and number of gastric polyps in the case group and the differences of the above characteristics between different genders were analyzed.Secondly,univariate difference analysis was carried out for the study variables involved in the case group and the control group.Univariate and multivariate binary Logistic regression analysis was used for risk factors.The patients with gastric polyp were divided into two subgroups,single gastric polyps and multiple gastric polyps,and the risk factors of the two groups were analyzed.Results1.In terms of distribution characteristics,the proportion of different parts of gastric polyps from high to low were antrum(53.78%),body of stomach(14.59%),duodenum(5.99%),fundus of stomach(5.87%),cardia(4.36%),pylorus(4.01%),Angle of stomach(2.09%)and other conditions of multiple parts and unclear;Gastric polyps were mainly single,accounting for 84.8% of all patients with gastric polyps,and multiple gastric polyps accounted for 15.2% of all patients with gastric polyps.In terms of the overall site distribution,there was a statistical difference in the distribution of gastric polyps,but the two groups of single gastric polyps and multiple gastric polyps were mainly concentrated in the antrum.The distribution of gastric polyps in the single gastric polyp group was higher than that of multiple gastric polyps,and the distribution ratio of multiple gastric polyps in the duodenum was higher than that of single gastric polyps.In the patients with gastric polyps,females accounted for 53.5% of the total population of gastric polyps,while males accounted for 46.5%.There was no statistical difference in the distribution of single and multiple gastric polyps in patients with different genders,but there was a statistical difference in the distribution of polyps in the fundus of stomach and the body of stomach.2.Single factor difference analysis results show that,Gender,age,education,marriage,total family income,BMI,total cholesterol,triglyceride,high density lipoprotein cholesterol,low density lipoprotein cholesterol levels,systolic blood pressure,diastolic blood pressure measurements,fruit and vegetable consumption time,fast eating habits,smoking,history of hypertension,hypersensitive C-reactive protein,glucose,creatinine,pepsinogen I,pepsinogen There were statistically significant differences in Ⅱ,pepsinogen Ⅰ/Pepsinogen Ⅱ and Helicobacter pylori positive infection(P<0.05).The two groups were compared in nationality,occupation,hot eating habits,preference for fried food,alcohol consumption,insomnia,lethargy,diabetes,gallbladder disease,history of peptic ulcer,family history of digestive system tumor,bile reflux,erosion,alanine aminotransferase,aspartate aminotransferase,total bilirubin,direct bilirubin,indirect bilirubin and urea.There was no statistical significance(P>0.05).3.Univariate logistic regression analysis indicated that male,older age,lower total family income,BMI≥24,shorter vegetable and fruit time,smoking,hypertension,no fast eating habit,elevated pepsinogen Ⅱ decreased pepsinogen Ⅰ/pepsinogen Ⅱ,Helicobacter pylori infection,elevated hypersensitive C-reactive protein,abnormal renal function indicators were the risk factors for gastric polyps Factor.4.Multiple Logistic regression analysis indicated that male,older age,shorter time to eat fruits and vegetables,elevated pepsinogen Ⅱ,Helicobacter pylori infection,and history of hypertension were all independent risk factors for gastric polyps.5.Univariate Logistic regression analysis of single gastric polyp suggests:Male,older age,unmarried,lower family income,BMI≥24,dyslipidemia,less time to eat vegetables and fruits,no fast eating habit,smoking,increased PⅡ,decreased PGR,hypertension,helicobacter pylori infection,increased hypersensitive C-reactive protein,abnormal renal function index were risk factors for single gastric polyps(P < 0.05).Multivariate Logistic regression analysis indicated that male,older age,shorter time to eat fruits and vegetables,hypertension,increased PG-Ⅱ and Hp(+)were independent risk factors for single gastric polyp.6.Univariate Logistic regression analysis of multiple gastric polyps suggested that male,age "≥60 years old",low family income,increased PGⅡ,decreased PGR,and Helicobacter pylori infection were risk factors for multiple gastric polyps(P < 0.1).Multivariate Logistic regression analysis indicated that male,lower PGR,age " ≥60 years old" and low family income were independent risk factors for multiple gastric polyps.Conclusion1.Gastric polyps are still predominantly solitary gastric polyps,with a higher proportion of women than men among patients with gastric polyps;in terms of site distribution,polyps in the gastric sinus are the most common;2.The occurrence of gastric polyps was associated with gender,age,marriage,total household income,BMI,fruit and vegetable time,smoking,hypertension,fast food habits,pepsinogen level,H.pylori infection,elevated ultrasensitive C-reactive protein,and abnormal renal function indexes,among which age and pepsinogen level had more obvious effects on the occurrence of gastric polyps;3.Dyslipidemia and overweight,time of eating vegetables and fruits,smoking,history of hypertension,elevated PG II,elevated ultrasensitive C-reactive protein level,and abnormal renal function were associated with the occurrence of solitary gastric polyps;decreased PGR was associated with the occurrence of multiple gastric polyps. |
