Objective: We investigated the characteristics and factors influencing eye emotion recognition in selflimited epilepsy with centrotemporal spikes complicated with electrical status epilepticus during sleep.Method:Se LECTS 160 patients treated in the outpatient and inpatient departments of Anhui Children’s Hospital from September 2020 to January 2022.According to the video EEG Spine Slow Wave Index(SWI),patients with SWI<50% were classified into typical Se LECTS(n=79),and patients with SWI≥50% were classified into the group with ESES(n= 81).The basic emotion discrimination task in the eye area and the complex emotion discrimination task system in the eye area were respectively compared with the healthy control group.The sick group and the healthy control group were matched in terms of gender,age and education level.The correlation between the characteristics of emotional discrimination disorder in the eye area and the clinical influencing factors was analyzed in ESES group.Result:(1)Se LECTS: Emotion recognition Results from the eye region:Patients with Typical Se LECTS identified sadness and fear [M(Q25,Q75): Sadness 17.0(16.0,17.5),fear 15.0(14.5,16.0)] lower than those of healthy controls [M(Q25,Q75): Sadness18.0(15.0,19.0),fear 16.0(14.0,17.0)],the difference was statistically significant[sadness Z=-2.361,fear Z=-2.273,P <0.05)].However,in the identification performance of,happiness disgust,surprise and anger [M(Q25,Q75): happiness 20.0(18.5,20.0),disgust 16.0(16.0,17.0),surprise 19.0(18.0,20.0),anger 18.0(16.0,18.0)],showed no significant difference compared with the healthy control group [M(Q25,Q75): Joy19.0(18.5,20.0),disgust 17.0(16.0,19.0),surprise 19.0(18.0,20.0),anger 18.0(16.0,19.0)],[happiness Z =-1.397,disgust Z =-1.799,surprise Z =-0.851,anger Z=-0.879,P all > 0.05)](2)ESES:Emotion recognition Results from the eye region:ESES group had lower scores on emotion recognition of sadness,fear,disgust and surprise [M(Q25,Q75):sadness 16.0(14.0,18.0),fear 15.0(14.0,16.0),disgust 16.0(14.0,18.0),surprise 18.0(16.0,19.0)] than control group [M(Q25,Q75): sadness 18.0(15.0,19.0),fear 16.0(14.0,17.0),disgust 17.0(16.0,19.0),surprise 19.0(18.0,20.0)],the differences were statistically significant [sadness Z=-2.742,fear Z=-2.419,disgust Z=-2.029,surprise Z=-2.072,all P <0.050)];However,the emotion recognition scores of happiness and anger [M(Q25,Q75): happiness 20.0(17.0,20.0),anger 18.0(16.0,19.0)] and control group [M(Q25,Q75):19.0(18.0,20.0)and anger 18.0(16.0,19.0)],There was no significant difference between happiness(happiness Z=-0.433,anger Z=-1.097,all P>0.05).(3)Correlation analysis between emotional discrimination disorder in eye area and clinical factors in children with ESES group:Univariate logistic analysis showed that the score of eye recognition for sadness in the ESES group was affected by age of onset,SWI,ESES duration and number of seizures.The score of eye recognition for fear was mainly affected by SWI,while the score of eye recognition for disgust was affected by SWI and number of seizures.The surprised eye emotion recognition score was mainly affected by the number of seizures.Variables with P<0.1 are used as independent variables of multivariable ordered logistic regression.Multivariate logistic analysis showed that sadness emotion recognition was mainly affected by SWI and ESES duration,while disgust was mainly affected by SWI.Conclusion: The Se LECTS group showed impaired emotion(sadness and fear)recognition function in the eye area.Se LECTS complicated with ESES was associated with more extense emotional(sadness,fear,disgust,and surprise)recognition impairment in the eye region.The higher the SWI,the younger the onset age and the longer the duration of ESES,while the more the number of seizures,the more serious the impairment of emotional recognition function in the affected eye area.
