| Objective:To observe the distributions of TCM syndromes of patients with renal parenchymal hypertension from the outpatient and inpatient of department of Nephrology of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine.To find out the most common TCM syndromes and to observe the clinical efficacy of auricular needle-embedding as an adjuvant treatment.Methods:168 patients with renal parenchymal hypertension were selected from the outpatient and inpatient department of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine,The most common TCM syndromes were determined by questionnaire surveys and TCM consultations.Then 64 patients were selected and the patients were divided in half by using the random number table method(treatment group and control group).The treatment group was given conventional and auricular needle-embedding treatment and the control group was given conventional treatment for 2 weeks.Clinical indexes about systolic blood pressure,diastolic blood pressure,mean arterial pressure and TCM syndrome scores were collected and were compared between two groups,and changes in serum creatinine,blood urea nitrogen,glomerular filtration rate,cystatin-C,β2-microglobulin and 24 h urine protein quantification were observed before and after treatment.Finally we evaluated the clinical efficacy of auricular needle-embedding as an adjuvant treatment of renal parenchymal hypertension.Results:1.The frequency of TCM syndromes distribution of 168 patients with renal parenchymal hypertension in descending order: Yin deficiency and Yang excess(45%),phlegm-dampness retention(29%),hyperactivity of liver-fire(17%),and yin-yang deficiency(9%).2.The levels of systolic blood pressure,diastolic blood pressure and mean arterial pressure after treatment in both groups were lower than those in the same group before treatment(P<0.05),and the levels of treatment group were lower than that of control group(P<0.05).3.The levels of blood creatinine,urea nitrogen,cystatin-C andβ2-microglobulin after 2-week treatment were lower than those before treatment in same group(P<0.05),and the level of glomerular filtration rate was higher than that before treatment(P<0.05).The levels of blood creatinine and blood urea nitrogen in the treatment group were lower than those in the control group,and the level of glomerular filtration rate was higher than that in the control group(P<0.05),and the levels of cystatin-C andβ2-microglobulin were no significant difference compared with those in the control group(P>0.05).4.The level of 24 h urine protein quantification after 2-week treatment was lower than that before treatment in the same group(P<0.05),and there was no significant difference between two groups after treatment(P>0.05).5.The total TCM syndrome scores of the two groups after treatment were lower than those before treatment in the same group(P<0.05),and the scores of treatment group was lower than those of the control group(P<0.05).6.The total effective rate of TCM syndrome in the treatment group was86.67%,which was better than that of 76.67% in the control group(P<0.05).7.The total effective rate of blood pressure in the treatment group was83.33%,which was better than that of 73.33% in the control group(P<0.05).8.The clinical effective rate of the treatment group was 86.67%,which was superior to that of control group of 73.33%(P<0.05).Conclusion:1.Auricular needle-embedding therapy helps to relieve the clinical symptoms of patients with renal parenchymal hypertension.2.Auricular needle-embedding therapy helps to control blood pressure in patients with renal parenchymal hypertension.3.Auricular needle-embedding therapy can improve the TCM syndrome scores of patients with renal parenchymal hypertension and reduce the blood creatinine,blood urea nitrogen levels and improve glomerular filtration rate. |
