Since the reform and opening up,China ’s economic level has been significantly improved,and the living standards of Chinese people have undergone tremendous changes.However,the aging of the population is becoming more and more serious,the people ’ s demand for health services will gradually increase.Medical and health services are an extremely important part of social public services and social public management.At present,the distribution of medical resources in China ’s medical system is extremely uneven,and there is a huge gap in medical and health services among medical institutions at all levels : the medical quality of tertiary medical institutions is high,but the medical resources are limited and overcrowded,which generally reflects the problems of difficult and expensive medical treatment;some medical resources of secondary hospitals are surplus,but there is still room for optimization and upgrading of health services.In this paper,by comparing the medical service volume,bed use efficiency,service effect,talent reserve,medical quality control evaluation results and two-way referral data of the secondary public hospital X hospital in the medical association in 2015 and 2020,it is found that the medical and health services have been greatly improved.However,in terms of the two-way referral data reflecting hierarchical diagnosis and treatment,patients still choose to go to the tertiary hospital for the first diagnosis,rather than first go to the primary health service center and then transfer to the community health service center for rehabilitation and follow-up.As a result,the medical and health services provided by the secondary hospitals in the medical consortium are hindered by the failure to establish a new order of hierarchical diagnosis and treatment.This paper uses collaborative governance theory and principal-agent theory,and draws lessons from foreign medical consortium model,internal and external aspects of medical consortium to discuss the problems and reasons of medical and health services in X hospital in medical consortium.The problems and reasons are unclear positioning,inadequate implementation of hierarchical diagnosis and treatment system,poor information sharing,lack of role of middle layer in medical system and insufficient service ability.Combined with theoretical discussion,an effective collaborative governance system and effective incentive mechanism are established,and the optimization strategy of medical and health services in secondary public hospitals in medical consortium is put forward.Different levels and different types of medical institutions should establish a clear goal,clear responsibilities,fair and effective division of labor and cooperation mechanism.The government should establish a guidance and incentive mechanism with consistent responsibilities and rights,so that the medical resources in the medical consortium area can be truly and effectively shared,promote the improvement of grassroots service capacity,and realize the hierarchical diagnosis and treatment order of first diagnosis,two-way referral,acute and chronic treatment,and upper and lower linkage.Strengthen the role of the middle layer of the secondary public hospitals,and better serve the three-level medical system of the medical consortium and the social patients.It is hoped that through the joint efforts of the government and the member units in the medical consortium,the functional positioning of the secondary public hospitals and their role in the hierarchical diagnosis and treatment will be clarified,and the medical and health services of the secondary public hospitals in the medical consortium will be improved,so that the government can make full use of the medical resources at the same level,promote the reform of the medical consortium,and implement the healthy China strategy.
