Professor Liu Zhongyong’s Clinical Experience In The Treatment Of Ventricular Premature Contractions

Research purpose:Premature ventricular contractions(hereinafter referred to as PVCs)are a common type of arrhythmia that occurs when the heart beats irregularly.Professor Liu Zhongyong,a renowned traditional Chinese medicine(TCM)practitioner,has been treating PVCs for many years and has accumulated a wealth of clinical experience in this area.By collecting and sorting out the clinical case data of Professor Liu Zhongyong in the treatment of PVCs,using data mining technology,Combined with relevant documents published by Professor Liu Zhongyong’s team,to sorted out and analyzed the clinical experience of Professor Liu Zhongyong drug composition in the treatment deeply.The objective of this study is to analyze Professor Liu Zhongyong’s clinical experience in treating PVCs,to extract his treatment characteristics,further more,to broaden the thinking of clinical differentiation and treatment of PVCs.Research methods:The 206 case data of PVCs patients treated by Professor Liu Zhongyong in the outpatient and inpatient departments of the Affiliated Hospital of Jiangxi University of Chinese Medicine from August 2020 to November2022 were collated and summarized,including drugs,nature and flavor,efficacy,symptoms,tongue and pulse.First of all,the above data were standardized,and then,entered the above information into the ancient and modern medical case cloud platform(V2.3.5).By using the algorithms and data analysis functions of the software platform,selecting frequency statistical analysis,association rule analysis,cluster analysis,and other research methods to uncover the high-frequency drugs,drug combinations,core prescriptions,symptom patterns,and patterns between symptoms and medications of Professor Liu Zhongyong’s treatment of PVCs,and to summarize and refine Professor Liu Zhongyong’s understanding of the etiology and pathogenesis of PVCs and his clinical treatment characteristics.Research results:1.The results of the study on the pattern of medication used by Professor Liu Zhongyong in the diagnosis and treatment of PVCs showed that: the drugs with higher frequencies were salvia miltiorrhiza,tangerine peel,Mu Xing,malt,ligusticum wallichii,Fu Shen,codonopsis and rice malt in that order;the four natures accounted for the most warm drugs,and the rest were used in the order of flat,slightly cold,cold,slightly warm,cool,and large cold drugs;among the five flavors,the frequency of sweet drugs ranked first,followed by bitterness,pungent,lightness and sourness decreases sequentially;among the channels,those belonging to the spleen,lung,stomach,liver and heart channels were more common;the efficacy of Chinese medicine is mainly divided into four categories,the first one :strengthening the spleen and stomach,the second category : soothing the liver and regulating qi,the third category : resolving turbidity and regulating the veins,and the fourth category : nourishing the heart and tranquilizing the mind;the drug association rules were selected the condition parameter with support > 0.3,and confidence > 0.6,have " Mu Xing-salvia miltiorrhiza"," tangerine peel-salvia miltiorrhiza","tangerine peel-Mu Xing "," rice malt-malt","tangerine peel-malt "," malt-salvia miltiorrhiza " etc.;clustering combinations include:the first group:Ligusticum Wallichii,Pueraria,Astragalus;the second group : Codonopsis,Atractylodes,Fructus Amomi,Poria Cocos,Yam,Lotus Leaf,Coix Seed,Lentil;the third group : Malt,Rice Malt,Radix Pseudostellariae,Radix Ophiopogonis,Schisandra,Angelica Sinensis,Fu Shen,Polygala Tenuifolia,Suan Zaoren;the fourth Group : Tangerine peel,Salvia miltiorrhiza,Mu Xiang,rhizoma pinellinae praeparata,Scutellaria,Magnolia officinalis,Perilla stem,prepared licorice,radix curcumae,loquat leaves,radix bupleuri,radix paeoniae rubra.2.The results of the study on the symptom pattern of PVCs identified by Professor Liu Zhongyong showed that the symptoms with higher frequency were : palpitation,chest tightness,dizziness,poor sleep,dry mouth,fatigue,emotional impact,poor appetite,loose stools,sticky stools,and excessive eye discharge;the tongue and pulse signs were mainly light red tongue,dark red tongue,red tongue,side tooth marks,white and greasy coating,thin white coating,thin string pulse,and thin pulse.The symptom association rule was selected with support degree > 0.2,and confidence degree >0.6,in addition to poor sleep-palpitation,dizziness-palpitation,chest tightness-palpitation,dry mouth-palpitation,emotional impact-palpitation,fatigue-palpitation,poor appetite-palpitation,loose stool-palpitation;clustering combinations include: group 1:shortness of breath,chest pain;group 2: emotional impact,chest and hypochondrium tightness,dry mouth,bitter mouth;group 3 : lumbago,headache;group 4 : abdominal distension,cloudy urine,excessive eye discharge,loose stools,sticky stool,numbness of limbs,fatigue,poor appetite,palpitations,poor sleep,chest tightness,and dizziness.The associations between drugs and symptoms include: palpitations--salvia miltiorrhiza,tangerine peel,Mu Xing,malt,chest tightness--salvia miltiorrhiza,tangerine peel,Mu Xing,dizziness--salvia miltiorrhiza,tangerine peel,Mu Xing,ligusticum wallichii,poor sleep--salvia miltiorrhiza,tangerine peel,Mu Xing,Fu shen,poor appetite--tangerine peel,ligusticum wallichii,salvia miltiorrhiza,malt,Mu Xing,emotional impact--radix bupleuri,salvia miltiorrhiza,tangerine peel,radix curcumae,Mu Xing,Fu shen,dry mouth--salvia miltiorrhiza,tangerine peel,Mu Xing,fatigue--salvia miltiorrhiza,tangerine peel,ligusticum wallichii,astragalus,Mu xiang,loose stools-Salvia miltiorrhiza,tangerine peel,Mu xiang,ligusticum wallichii,Atractylodes,Coix seed,chest and hypochondrium tightness--radix bupleuri.Research conclusion:1.Professor Liu Zhongyong’s understanding of the etiology of PVCs.He emphasizes the nourishing effect of the spleen and stomach in generating qi and blood for the heart,and stresses that spleen deficiency is the intrinsic cause of PVCs.he believes that turbid evil is an important causative factor and pathological product of PVCs,so it is the pathogenic marker of PVCs.He believes that the characteristics of the modern social environment cause most people to be emotionally unstable and live a dysregulated life,resulting in liver qi disorder,which is the cause of the disease.he believes that collateral channels are the approach of PVCs,and deficiency of qi and yin is the important pathogenesis of this disease.2.Professor Liu Zhongyong’s clinical treatment features: clinical treatment pays attention to strengthening the spleen,soothing the liver,resolving turbidity,and nourishing yin.Embodying the clinical characteristics of Professor Liu Zhongyong in differentiating and treating PVCs : the spleen and stomach are essential,tonifying and transporting should be appropriate,paying attention to the harmonious of Qi and blood,regulating the liver firstly,invigorating qi and promoting blood,resolving turbidity and regulating the veins,protecting qi and nourishing yin,and nourishing the heart and tranquilizing the mind.Professor Liu Zhongyong used his decades of clinical experience to develop his own clinical medical technology and received good clinical efficacy in the clinical application of the dialectical addition and subtraction treatment PVCs,such as the formula for strengthening the spleen and resolving turbidity and restoring rhythm,dredging the liver and strengthening the spleen and harmonizing blood,the formula for activating and resolving turbidity and regulating the veins,and the formula for invigorating qi and nourishing the yin and calming the heart.
