A Meta-analysis Of Clinical Outcomes And Complications Of Ankle Arthroplasty And Fusion For Traumatic Ankle Arthritis

Abstract/Summary:
Objective:A systematic analysis of the outcomes and complications of total ankle arthroplasty versus ankle fusion for the treatment of traumatic ankle arthritis was performed using a meta-analysis.Methods:A comprehensive literature search was conducted by searching Pub Med,Cochrane Library,Embase,CNKI,VIP and Wanfang databases for case-control studies of the Chinese and English literature on the correlation between prosthetic ankle arthroplasty and ankle fusion for traumatic ankle arthritis at home and abroad since the establishment of the database until October 2022,and retrospectively citing and referencing the literature.Screening was performed strictly according to the established inclusion and exclusion criteria,and the risk of bias and bias analysis of the included literature that met the criteria were evaluated,and relevant data were extracted for Meta-analysis using Rev Man 5.3 software.Results:A total of 12[6-17]literatures,6[7,8,9,13,14,16]retrospective cohort studies,and 6[6,10,11,12,15,17]randomized controlled trials with a total of 793 cases were included in this study.Compared with ankle fusion,patients with artificial ankle replacement had lower operative time(MD=-44.95,95%CI:-79.31-10.60,P<0.01),higher SF-36 scores(MD=10.22,95%CI:9.02-11.42,P<0.01),and higher Kofoed scores(MD=6.83,95%CI:4.13-9.53,P<0.01),higher AOFAS score(MD=5.10,95%CI:2.10-8.10,P<0.01),and lower postoperative claudication(MD=0.17,95%CI:0.07-0.44,P<0.01).And,there was no statistical significance between the two groups in terms of VAS score(MD=-0.92,95%CI:-1.88-0.04,P=0.06)and postoperative complications(MD=-2.74,95%CI:0.88-8.50,P=0.08).However,ankle fusion had a significant advantage over ankle arthroplasty in terms of intraoperative bleeding(MD=42.91,95%CI:2.55-83.26,P=0.04)and number of postoperative revisions(MD=3.61,95%CI:1.60-8.19,P<0.01).Conclusion:Compared to ankle fusion,patients with artificial ankle replacement have the advantages of shorter operative time,better postoperative sensation,and faster recovery of ankle function,but significantly more intraoperative bleeding and postoperative revisions.
