Objective: To observe the degree of improvement of anxiety,depression and neurological deficits in patients with post-stroke Anxiety-depressive State(liver-depression and spleen-deficiency type)by applying Peitu Yimu Prescription combined with acupuncture on the basis of basic medical treatment and oral sertraline,and to evaluate its efficacy and safety.So as to provide a new comprehensive treatment idea for post-stroke Anxiety-depressive State and to provide reference for new drug development.Methods: 96 patients with post-stroke Anxiety-depressive State of liver-depression and spleen-deficiency type were selected from January2022 to December 2022 in the outpatient clinic and ward of the rehabilitation department of Qinhuangdao Hospital of Traditional Chinese Medicine,and were divided into a control group and a treatment group of48 cases each using the random number table method.The control group was treated with oral sertraline hydrochloride tablets on the basis of basic medical treatment,while the patients were given health education and psychological counseling;in the treatment group,on top of the treatment in the control group,Peitu Yimu Prescription combined with acupuncture was given.Both groups were treated with 2 weeks as a course of treatment,and a total of 4 courses of treatment.Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)scores were performed before treatment,after 2 weeks,after 4 weeks,after 6 weeks and after 8 weeks,respectively.Neurological impairment assessment(NIHSS score)and TCM syndrome score were performed before and after treatment.Efficacy was assessed based on the integrated rate of change of TCM syndrome score.To comprehensively analyze the clinical efficacy of Peitu Yimu Prescription combined acupuncture of anxiety-depressive state after stroke and monitor the security of this scheme.The SPSS 25.0 statistical software was used for data analysis.Results: A total of 91 patients completed the trial in both groups,45 in the control group and 46 in the treatment group.There were no statistical differences in the general conditions(gender,age,course of disease,type of cerebrovascular disease),concomitant diseases(hypertension,type 2diabetes,hyperlipidemia,hyperhomocysteinemia,smoking,drinking),HAMD scores,HAMA scores,NIHSS scores,TCM syndrome scores(P>0.05),and were comparable.Post-treatment: 1.depression: HAMD scores of the two groups at the 2nd,4th,6th and 8th weeks of treatment were decreased compared with those before treatment(P<0.05),and the difference was statistically significant,between the two groups,there was no significant difference at the 2nd and 4th week of treatment(P>0.05),but significant difference at the 6td and 8th week of treatment(P<0.05 in the6 td week and P<0.01 in the 8td week).2.Anxiety: HAMA scores of the two groups at the 2nd,4th,6th and 8th weeks of treatment were decreased compared with those before treatment(P<0.01),and the difference was statistically significant,between the two groups,statistical significance in the 2nd,4th,6th and 8th weeks of treatment(P<0.01).3.Neurological function: NIHSS scores of the two groups at the 8th weeks of treatment were decreased compared with those before treatment(P<0.01),and the difference was statistically significant,between the two groups,statistical significance after the 8th weeks of treatment(P<0.01).4.TCM syndrome score: TCM syndrome scores of the two groups after the 8th weeks of treatment were decreased compared with those before treatment(P<0.01),and the difference was statistically significant,between the two groups,statistical significance after the 8th weeks of treatment(P<0.01).Before treatment,there was no significant difference in the scores of main symptoms between the two groups(P>0.05),which was comparable.After treatment,the scores of main symptoms in both groups decreased,and the difference was statistically significant(P<0.01).After treatment,there was no significant difference between the two groups in terms of Depression and Irritability(P>0.05),but significant difference between the two groups in terms of Rib-side pain,Torpid intake and Loose stool(P<0.01).Before treatment,there was no significant difference in the scores of secondary symptoms between the two groups(P>0.05),which was comparable.After treatment,the scores of secondary symptoms in both groups decreased,and the difference was statistically significant(P<0.01).After treatment,there was no significant difference between the two groups in terms of Insomnia(P>0.05),but significant difference between the two groups in terms of Sighing,Flatus,Abdominal distention and Shortness of breath(P<0.01 in Sighing,Flatus,Abdominal distention and P<0.05 in Shortness of breath).5.Comparison of curative effect: The total effective rate was 84% in the control group and 96% in the treatment group,the difference was statistically significant(P<0.05).6.Security: None of the 91 patients who completed the trial had a significant adverse effect.No abnormalities were found in the associated security indicators,indicating good security.Conclusion: The combination of Peitu Yimu Prescription with acupuncture for the treatment of post-stroke Anxiety-depressive State in liver-depression and spleen-deficiency type can reduce patients’ TCM syndrome scores,alleviate depression and anxiety,improve neurological function and enhance quality of life,with better clinical efficacy than Western medicine treatment alone,and less side effects,better safety and higher patient compliance.
