| Objective:To investigate the prevalence and influencing factors of diabetes and hyperuricemia in the population aged 60 years and above in Anning city,Yunnan province,and to try provide a theoretical basis for the prevention and treatment of chronic diseases in the elderly in Anning city.Methods:From September 2019 to January 2020,we used multi-stage stratified cluster sampling method in this cross-sectional study method,to select 9447 the elder from anning city among 72 natural villages,using questionnaires on the object of study general demographic information including age,gender,acquisition research object fasting blood glucose,blood uric acid,high density lipoprotein cholesterol,low density lipoprotein cholesterol and other biochemical indicators.According to the metabolic status of glucose,the subjects were divided into diabetic group,impaired fasting glucose group and normal blood glucose group.According to the blood uric acid metabolism status,the subjects were divided into hyperuricemia group and normal uric acid group,and the influencing factors of diabetes and hyperuricemia in the local elderly population were analyzed.Results:A total of 9447 subjects were included in the study,including 4222 males(44.69%)and 5225 females(55.31%),aged 72.02±6.14 years.There were 8836(93.53%)the Han nationality and 611(6.47%)ethnic minorities.Most of the respondents were in the 60~70 age group(43.00%)and 70~80 age group(43.78%).The respondents were from 5 different districts in Anning City,with the largest number of respondents in Lianran district(35.76%)and the least number of respondents in Qinglong Town(5.97%).The prevalence of diabetes in Anning City was 17.25%(95%CI:16.49%~18.02%)and the prevalence of impaired fasting glucose was 13.41%(95%CI:12.72%~14.10%)among the elderly aged 60 years and above.There were 25.77%(420/1630)patients with previous diabetes and 74.23%(1210/1630)patients with newly discovered diabetes.The prevalence of diabetes in Han population was higher than that in minority population(17.46%vs14.24%,P<0.05).The elder in the 60~70 age group has the highest incidence of diabetes mellitus(696/4062,17.14%)and impaired fasting glucose(584/4062,13.49%),the 90~100 age group years has the lowest mellitus(10/82,12.20%)and impaired fasting glucose prevalence(9/82,10.97%).The prevalence of diabetes(220/761,28.91%)and impaired glucose regulation(140/761,18.40%)were the highest in obese people,and the prevalence of diabetes(60/688,8.72%)and impaired glucose regulation(60/688,8.72%)were the lowest in underweight people.The prevalence of diabetes and impaired glucose regulation increased with the increase of BMI(Ptrend<0.05).The prevalence of diabetes and impaired fasting glucose in the elderly from the higher economic development level was higher than that in the elderly from the middle economic development level(P<0.05).The results of multivariate logistic regression analysis showed that there was a cross-layer interaction between economic development level and ethnic minorities on the prevalence of diabetes mellitus aged 60 years and above in Anning city(OR=0.307,95%CI 0.190~0.494).Ethnic minorities living at a higher level of economic development had a lower prevalence of diabetes mellitus.Dyslipidemia(OR=1.822,95%CI 1.100~3.021),overweight(OR=2.749,95%CI 1.612~4.687)and age(OR=1.562,95%CI 1.102~2.214)were all risk factors for diabetes.The prevalence of hyperuricemia was 18.97%(95%CI:18.18%~19.76%)in the elderly aged 60 years and above in Anning City,Yunnan Province.The prevalence of hyperuricemia in males was higher than that in females(30.89%vs9.34%,P<0.05).The prevalence of hyperuricemia was the highest in the 80~90 year old age group(288/1167,24.68%),and the prevalence of hyperuricemia increased with the increase of age group(Ptrend<0.05).The prevalence of hyperuricemia in obese population was the highest(226/761,29.70%),and was the lowest(60/688,11.77%)in underweight population,and the prevalence of hyperuricemia in population showed an increasing trend with the increase of BMI(Ptrend<0.05).The prevalence of hyperuricemia in elderly people from higher economic development areas was higher than that in elderly people from middle economic development areas(P<0.05).Multivariate logistic regression analysis showed that there was a cross-layer interaction between economic development level and ethnic minorities on the prevalence of hyperuricemia in the elderly aged 60 years and above in Aning City(OR=0.270,95%CI 0.196~0.370).Ethnic minorities living at a higher level of economic development had a lower prevalence of hyperuricemia,gender(OR=7.379,95%CI 4.424~12.311),BMI(OR=1.082,95%CI 1.013~1.155),dyslipidemia(OR=2.286,95%CI 1.398~3.738)and abnormal CRE(OR=2.997,95%CI1.798~4.994)was a risk factor for hyperuricemia.Conclusions:1.The prevalence of diabetes in the elderly aged 60 years and above in Anning City of Yunnan Province was 17.25%(95%CI 16.49%~18.02%),which was low in the whole country but high in Yunnan Province.There is a cross-layer interaction between the level of economic development and ethnic minorities on the prevalence of diabetes in the elderly.The ethnic minorities living in the areas with higher economic development level have a lower prevalence of diabetes,and the main risk factors include dyslipidemia,overweight and age.It is suggested that the health management of diabetes and its risk factors should be strengthened and the tertiary prevention of diabetes should be done well.2.The prevalence of hyperuricemia in the population aged 60 years and above in Anning City of Yunnan Province was 18.97%(95%CI 18.18%~19.76%),which was low in the whole country.There was a cross-layer interaction between economic development level and ethnic minorities on the prevalence of hyperuricemia in the elderly.The main risk factors for hyperuricemia include male sex,BMI,dyslipidemia,and abnormal creatinine.It shows that the health problem of hyperuricemia in the elderly population in Anning cannot be ignored,and the health management of hyperuricemia should be carried out as soon as possible. |
