Objective: This study evaluated the effect of "Jianpi Yangrong Prescription" on the incidence and grade of myelosuppression,quality of life and TCM symptoms of patients with breast cancer after postoperative chemotherapy,as well as the effects on liver and kidney function and urine routine.To provide objective data support for the clinical promotion of "Jianpi Yangrong Decoction" in the prevention and treatment of bone marrow suppression of breast cancer,and to provide clinical research basis for the treatment of breast cancer with TRADITIONAL Chinese medicine.Methods: 64 patients in the outpatient department and inpatient department of The Affiliated Hospital of Hunan Academy of Traditional Chinese Medicine who met the inclusion criteria were randomly divided into experimental group and control group with 32 cases in each group.Preparation of invigorating spleen nourishing rong Fang traditional Chinese medicine decoction(Astragalus membranaceus,Codonopsis pilosula,Atractylodes atractylodes,etc.).The control group was treated with shengxuebao granule from the first day of chemotherapy,twice a day,each time a packet of 200 ml water,for 14 days.In the experimental group,jianpi yangrong decoction was taken orally from the first day of chemotherapy,200 ml each time,twice a day.Routine blood examinations were performed in the two groups 1 day before chemotherapy and 3,7,10 and 14 days after chemotherapy,respectively.The levels of white blood cells,neutrophils and platelets were compared between the two groups after treatment(day 3,7 and 14 after chemotherapy).The incidence and grade of myelosuppression were compared between the two groups.Efficacy was evaluated by quality of life score and TCM syndrome score before and after chemotherapy.The liver,renal function,urine routine and adverse reactions during treatment were compared between the two groups before and after chemotherapy.Results: A total of 64 patients were included in this study,of which 56 were actually completed,including 28 cases in the treatment group and 28 cases in the control group.There were 8 cases who were discharged from the hospital during the treatment and did not review the relevant indicators at the corresponding time.The lack of clinical data collection led to the shedding of the cases,resulting in 4 cases in the control group and 4cases in the treatment group.There was no significant difference in general data between the two groups(P > 0.05),indicating comparability.(1)Comparison of efficacy:(1)There were no significant differences in leukocytes and neutrophils before chemotherapy,which were comparable(P > 0.05);(2)On the3 rd and 7th day after chemotherapy,the levels of white blood cell and neutrophil were decreased in both groups,and the counts of white blood cell and neutrophil in the treatment group were slightly higher than those in the control group,with no statistical significance(P > 0.05).On the 10 th day after chemotherapy,the levels of white blood cells and neutrophils in the control group decreased to the lowest level,and the level in the treatment group increased,which was significantly higher than that in the control group(P<0.05).On the 14 th day of chemotherapy,white blood cells and neutrophils in both groups recovered,and the increasing trend in the treatment group was more obvious than that in the control group,and the recovery value was also higher than that in the control group(P<0.05).There was no significant difference between the recovery value and that before chemotherapy(P >0.05),while the observation group was still lower than that before chemotherapy(P<0.05).(3)The probability of myelosuppression was 42.85% in the treatment group and 60.71% in the control group,the difference was not statistically significant(P >0.05),but there was a significant difference in the grade of myelosuppression between the two groups,the treatment group mostly concentrated on grade I myelosuppression,while the control group mostly concentrated on grade II and above myelosuppression.(2)There was no significant difference between the TCM syndrome scores of the two groups before treatment,which was clinically comparable(P>0.05).After chemotherapy,the TCM syndrome scores of the two groups increased,but 14 days after treatment,the TCM syndrome scores of the treatment group were significantly lower than that of the control group,the difference was statistically significant(P<0.05).The TCM syndrome score was significantly lower than that of the most severe myelosuppression 7 days after chemotherapy.(3)There was no statistical difference in KPS scores between the two groups before treatment(P>0.05).After treatment,KPS scores in the treatment group were significantly higher than those in the control group(P<0.05),and there was no difference before treatment(P>0.05).KPS scores in the control group decreased compared with those before treatment(P<0.05).(2)Comparison of safety: There were no obvious adverse reactions related to treatment agents in the two groups.The overall vital signs of the patients were stable,and there were no significant differences in liver,kidney and urine routine after treatment compared with before treatment.Conclusion: Tonifying spleen and nourishing Rong can effectively reduce the incidence and degree of bone marrow suppression in breast cancer patients with qi and blood deficiency syndrome after postoperative chemotherapy.It has a significant effect on leukopenia and neutropenia,and is effective in improving the quality of life and alleviating TCM symptoms.It is worthy of promotion and application in clinical practice,and further study to expand the scope of use.
