Clinical Efficacy Observation And Network Pharmacology Study Of Yiqi Tongmai Decoction In The Treatment Of Unstable Angina Pectori

Purpose: To observe the clinical effect of Yiqi Tongmai Decoction on patients with unstable angina pectoris(Qi deficiency and blood stasis type);To explore the network pharmacological mechanism of Yiqi Tongmai Decoction in the treatment of unstable angina pectoris,in order to provide micro level scientific basis for clinical treatment.Material and method:1.63 patients with unstable angina pectoris of qi deficiency and blood stasis type who were hospitalized in the first Department of Cardiology of the Affiliated Hospital of Liaoning University of traditional Chinese medicine from December 1,2020 to August 31,2021 and met the acceptance criteria were selected.They were divided into observation group and control group according to different treatment methods.Observation group: Western medicine standardized treatment combined with Yiqi Tongmai Decoction,control group: Western medicine standardized treatment,The two groups were treated for 4 weeks.The changes of angina pectoris score,blood lipid index and TCM symptom score were observed before and after treatment,and the efficacy of Yiqi Tongmai Decoction in the treatment of unstable angina pectoris(Qi deficiency and blood stasis type)was evaluated.2.Search and screen all chemical components of traditional Chinese medicine contained in Yiqitongmai Decoction through TCMSP database,use Uniprot database to predict the target of effective components of all drug flavors,and realize the standardization of target gene names;Query the disease targets of Unstable angina through genecards database;Venny diagram was used to find the potential therapeutic target of Yiqi Tongmai Decoction in the treatment of Unstable angina;Through string database and Cytoscape 3.7.2 software,PPI network diagram,drug active ingredient target gene network diagram of Yiqi Tongmai Decoction and Yiqi Tongmai Decoction active ingredient potential therapeutic target signal pathway network diagram were constructed respectively;Go function analysis and KEGG pathway enrichment analysis of target genes were carried out by using metascape database.Results:1.The symptom score of angina pectoris,serum TG and LDL-C in the two groups were significantly improved after treatment(P < 0.05);Compared with the above indexes after treatment,the observation group was better than the control group(P < 0.05).2.After treatment,the TCM symptoms of the two groups were improved(P < 0.05),and the improvement of TCM symptoms in the observation group was significantly better than that in the control group(P < 0.01).3.Yiqitongmai decoction may mainly treat Unstable angina through quercetin,luteolin,kaempferol,tanshinone IIA,7-methoxy-2-methylisoflavone,7-o-methylisoribol β-The drug components such as sitosterol,naringin,anthocyanin and baicalin may take effect mainly through AKT1,TP53,IL6,TNF,JUN,VEGFA and other targets.The key active components of Yiqitongmai Decoction acting on the signal pathway may be quercetin,luteolin,kaempferol,anthocyanin β-Sitosterol;The key target genes of Yiqi Tongmai Decoction acting on the signal pathway may be PTGS2,ESR1,NCOA2,PTGS1 and AR.4.Yiqi Tongmai Decoction plays a major role in the treatment of unstable angina pectoris through lipid and atherosclerotic signal pathway,and estrogen signal pathway and ovarian steroid production signal pathway may also play a certain role.Conclusion:1.Yiqi Tongmai Decoction can improve the symptoms of angina pectoris in patients with unstable angina pectoris with Qi deficiency and blood stasis,and reduce serum TG and LDL-C.2.Yiqi Tongmai Decoction can improve the TCM symptoms of patients with unstable angina pectoris of qi deficiency and blood stasis type.3.Yiqi Tongmai Decoction has 182 effective components and 228 effective targets;1778targets of Unstable angina;Yiqi Tongmai Decoction has 10 kinds of core effective components in the treatment of Unstable angina;6 core targets;5 key active components in the signal pathway;There are five key target genes in the signal pathway.4.Yiqi Tongmai Decoction plays a major role in the treatment of Unstable angina through lipid and atherosclerotic signal pathway,and estrogen signal pathway and ovarian steroid production signal pathway may also play a certain role.
