Purpose:To mainly observe and evaluate the clinical efficacy of xiaoyao Granules on patients with Hashimoto’s thyroiditis,and to explore the possible immunomodulatory mechanisms from their effects on IFN-γ,IL-2,IL-6,and IL-10 inflammatory cytokines,and to provide referenceable immunological ideas and methods for the treatment of Hashimoto’s thyroiditis in traditional Chinese medicine.Methods:A total of 95 patients with Hashimoto’s thyroiditis with evidence of liver depression and spleen deficiency who met the inclusion criteria and attended the outpatient clinic of Professor Zhang Lan at the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from March 2020 to September 2021 were included,Oral Xiaoyao granules was given to the enrolled patients.A total of 12 weeks of treatment were observed,and the changes of relevant laboratory indicators such as thyroid function five items(FT3,FT4,TSH,TPOAb,TGAb),plasma inflammatory cytokines(IFN-γ,IL-2,IL-6,IL-10),and liver and kidney function and blood routine safety indicators were recorded before and after treatment.and changes in TCM syndrome scores and related adverse reactions.In this study,Excel tables and SPSS25.0 were used to collate monitoring data and statistical description analysis data,respectively.Results:1.A total of 95 cases were collected in this trial,9 cases were shed due to epidemic reasons and other factors,and 86 cases were finally included in the statistical analysis,with a treatment period of 12 weeks.2.Efficacy analysis: The TPOAb and TGAb indexes of Hashimoto’s thyroiditis patients after the treatment of Xiaoyao granules were both lower than those before treatment(P<0.01),and the difference before and after was obvious,The total effective rate of TPOAb decrease in the efficacy index was 87.2%,and the total effective rate of TGAb decrease was 88.37%,and the clinical remission rate achieved after treatment was high;FT3,FT4,and TSH differences were not statistically significant(P>0.05)and did not have an effect before and after treatment.3.TCM symptom scores: The application of Xiaoyao granules treatment could effectively reduce the patient’s primary and secondary symptoms such as goiter,fatigue and weakness,fullness in the chest,emotional and mental disorder,loose stools,tongue and pulse signs and tongue and pulse signs scores(P<0.01),and the total TCM symptom scores were significantly reduced(P<0.01),with statistical differences before and after treatment.4.Comparison of plasma inflammatory factors: serum pro-inflammatory factors IFN-γ,IL-2and IL-6 were significantly reduced in patients with Hashimoto’s thyroiditis after treatment(P< 0.01),and inflammation-inhibiting cytokine IL-10 was significantly higher than before treatment(P < 0.01).5.Safety aspect: no abnormal changes in blood routine,liver and kidney function and electrocardiogram were observed before and after treatment,which proved that the use of this drug has good safety,and Significant improvement in Hashimoto’s thyroiditis The main symptoms of liver depression and spleen deficiency in patients with Hashimoto’s thyroiditis.Conclusion:1.The dialectical treatment of Xiaoyao Granules can enhance the clinical treatment effectiveness of Hashimoto’s thyroiditis patients with liver depression and spleen deficiency.2.Xiaoyao Granules may enhance thyroid autoimmune function by reducing IFN-γ,IL-2,IL-6and increasing IL-10 levels to regulate TH1/TH2 balance,which can effectively antagonize the inflammatory response and correct thyroid damage caused by immune-related cytokine imbalance in patients with Hashimoto’s thyroiditis with a good safety profile.
