| Background:Non-suicidal self-injury(NSSI)is an intentional act of bodily harm that is not motivated by suicidal intent.Adolescence is a period of high prevalence of selfinjurious behavior,often accompanied by anxiety and depression,and has received much attention from researchers.A review of the literature shows that childhood trauma and alexithymia are important influences on self-injurious behavior,but it is unclear how childhood trauma acts on self-injury and through what pathways it affects selfinjury.And there is a paucity of research on interventions for patients with non-suicidal self-injury with childhood trauma.Research has demonstrated that acceptance commitment therapy(ACT)has been applied to depression and anxiety,obsessivecompulsive disorder,and self-injury to reduce depression and anxiety and improve psychological flexibility.Objective:To investigate the role of childhood trauma and alexithymia on adolescent nonsuicidal self-injury,and to provide support for a later intervention study to validate the effectiveness of acceptance commitment therapy for non-suicidal self-injury patients with childhood trauma.Methods:Questionnaires were administered to 287 study subjects who met the requirements according to the inclusion and exclusion criteria.Then,30 study subjects who met the inclusion and exclusion criteria were randomly divided into intervention and control groups.The intervention group underwent lithium carbonate treatment and acceptance commitment therapy intervention for a total of 8 weeks of ACT treatment,once a week for 60 minutes,and the control group was lithium carbonate treatment and interrogation.Patients were assessed with the following questionnaires at the pre-,mid-,post-,and 2-week post-intervention follow-up measures: general demographic information,ANSAQ,TAS-20,HAMD,HAMA,AAQ-Ⅱ.Results:1.The effect of childhood trauma on self-injury acts through two mediators: an independent mediator through experiential avoidance and a chain mediator through alexithymia and experiential avoidance.2.Treatment effects were significantly different between the intervention and control groups on HAMD,HAMA,ANSAQ,and AAQ-Ⅱ;time effects and the interaction of time and treatment were significantly different on HAMD,HAMA,ANSAQ,TAS-20,and AAQ-Ⅱ.Conclusions:1.Alexithymia and experiential avoidance play a chain mediating role in childhood trauma and self-injury.2.Acceptance commitment therapy can significantly reduce the occurrence of selfinjurious behaviors.3.Acceptance commitment therapy reduced experiential avoidance,increased psychological flexibility,and reduced depression,anxiety,and alexithymia. |
