Using the perspective of health sociology and based on the investigation in Hospital E,this study examines the effect of the action mechanism of "doctor-patient-social work tripartite linkage" constructed in the practice of medical social work in this hospital on the self-adaptation of diabetic patients. At present, most studies in the field of medical social work mainly use quantitative analysis methods to analyze the effectiveness of medical social work intervention in the out-of-hospital rehabilitation process of diabetic patients;other studies have examined the effective path of medical social work to help diabetic patients establish social support. However, the characteristics of diabetes determine that patients face the problems of long-term adaptation to the disease state and maintenance of treatment compliance, which means that medical social work needs to pay attention to the overall health of patients in "physical-psychological-social" aspects from the perspective of self-adaptation.In view of this, this study uses the research method of social work to collect and analyze data on the mechanism and process of medical social workers’ intervention in the self-adaptation of diabetic patients in Hospital E.It is found that the hospital has established a "medical-patient-social work tripartite linkage".mechanism of action. Therefore, this study attempts to present the formation process and operation of this linkage, and analyze its influence and realization mechanism on self-adaptation in diabetic patients. The research findings: First, when medical social workers solve the adaptation problem of diabetic patients, they find that multi-subject interaction has positive effects in reducing physical pain, improving psychological burden, and enhancing social support in diabetic patients.Action strategy of "medical-patient-social work tripartite linkage".Second, this linkage mechanism works by constructing an integrated prevention and treatment path of social prevention, individual treatment and out-of-hospital rehabilitation.Third, through three specific mechanisms(promoting good communication between doctors and patients, strengthening patients’ awareness and ability to make positive changes, and establishing a social support network),this path helps diabetic patients from a disease state to a healthy state, thereby enabling patients to achieve self-esteem.purpose of adaptation. However,in the process of helping diabetic patients to realize self-adaptation,this linkage mechanism still has problems such as the relative lack of authority of medical social workers, weak team strength, and poor “linkage” among doctors, patients, and social workers. Therefore, this study proposes countermeasures from the aspects of medical social work publicity, talent team building, and social support construction,so as to further improve the effectiveness of the "medical-patient-social work tripartite linkage" mechanism. In addition, this study is different from the simple elaboration of facts in existing studies, and provides a new analytical framework for explaining the mechanism of action of medical social work.
