| Dysarthria may have abnormalities in the ability,stability or accuracy required for breathing,Phonation,resonation,articulation,or Prosody of speech due to damage to the nervous system.The intonation of dysarthria is often described as "lack of Pitch variation,flat speech",especially in interrogative sentences,the abnormal intonation will significantly affect the speech intelligibility,reduce the efficiency of communication.The Purpose of this study was to explore the influence of different stimulus types on the intonation of interrogative sentences in Patients with dysarthria,and to develop the intervention strategy of interrogative intonation in Patients with dysarthria according to the experimental results,and to verify the clinical efficacy of the scheme through cross-subject multi-baseline research,with a view to improving the intonation function of patients and improving the speech communication function of Patients from the Perspective of transegmental Phoneme.In the first part,the effects of different stimulus types on the intonation output of interrogative sentences in patients with dysarticulation were studied by exploring the effects of exaggerated prosody stimulus,visual cue stimulus and exaggerated prosody combined with visual cue stimulus on the intonation output of interrogative sentences in patients with dysarticulation.Sixteen patients with dysarthria were selected to compare the intonation of interrogative sentences under different stimuli by using a single factor repeated measurement experimental design.The results showed that,compared with the no stimulus state,the intonation of interrogative sentences was changed in different degrees(P <0.05)after exaggerated prosody stimulus,visual cue stimulus,and exaggerated prosody combined with visual cue stimulus,and the boundary tone slope K and the average fundamental frequency slope K were significantly different under the three stimuli(P <0.001).Under the stimulation of exaggerated prosodic stimulation combined with visual cues,there were significant differences in the average speech fundamental frequency,boundary modulation fundamental frequency,standard deviation of speech fundamental frequency,dynamic range of fundamental frequency,average fundamental frequency slope K and boundary modulation fundamental frequency slope K(P < 0.05).Combined with the results of multiple analysis,the following results were obtained: Hyperprosody combined with visual cue improved the intonation of interrogative sentences more than hyperprosody and visual cue.Exaggerated prosodic stimuli had greater influence on interrogative intonation than visual cues.The second part was to design a structured intervention program for patients with dysarthria by combining exaggerated prosody with visual stimulation.In this study,3patients with unilateral upper motor neurone dysarticulation were selected to analyze the intonation of interrogative sentences by using the standard deviation of objective acoustic parameters,the slope of basal frequency K,the slope of average basal frequency K and the slope of basal frequency k in the single subject experiment.The results showed that the standard deviation of basal frequency,slope of basal frequency of boundary modulation K,slope of average basal frequency K,and intervention period of boundary modulation fundamental frequency of subject 1 were significantly different from baseline period(P < 0.001),and showed an upward trend combined with linear analysis.The standard deviation of fundamental frequency,slope of fundamental frequency K and average fundamental frequency K of subject 2 in the intervention period were significantly different from that in the baseline period(P <0.05),and linear analysis showed an upward trend.Compared with the baseline period,the standard deviation of basal frequency,the average slope of basal frequency K,and the slope of basal frequency with boundary modulation were significantly different(P< 0.001)in the intervention period,and the slope of basal frequency with boundary modulation was significantly different(P < 0.01).Combined with the linear analysis,the above results showed that the intonation parameters of each interrogative question in the 3 patients were improved to varying degrees after the intervention.The innovation of this study is to explore the effect of exaggerated prosody combined with visual cues on improving the intonation of interrogative sentences in patients with dysarthria,and to formulate corresponding comprehensive intonation intervention strategies,which are verified in clinical cases,providing a reference for clinical intervention of interrogative intonation in patients with dysarthria,which has certain practical significance and theoretical reference value. |
