Abstract/Summary:
The scale of the elderly population with stroke sequelae in China is constantly expanding.Due to a lack of health knowledge,elderly patients generally lack health awareness and health behaviors.Researches have shown that health behaviors are related to health states,thereby affecting the quality of life.The unhealth behaviors of elderly patients in various aspects affect their rehabilitation,therefore,helping elderly patients change unhealth behaviors is an urgent problem to be solved.The social worker relies on the professional advantages of holistic care and overall support to help others help themselves,providing support for the behaviors transformation of patients from physiological,psychological,and social aspects.This research uses the Transtheoretical Model of Change as the basic framework for intervention,combined with cognitive behavioral theory,and takes the elderly with stroke sequelae as an example to conduct a case work intervention study.The social worker uses different strategies targeted at the five stages of pre-intention,intention,preparation,action,and maintenance to promote change in the behaviors of the client.At each stage,the social worker focuses on the client’s self-efficacy and decision-making shifting towards a positive direction,helping him form various health behaviors,including following medical advice,eating healthy meals,scientific exercise,and controlling emotional impulsive behavior.During the pre-intention period,the social worker promotes the health awareness of client through health knowledge dissemination,awareness awakening,and emotional arousal.During the intention period,the social worker adopts techniques such as linking resources and self-evaluation to help the client understand the importance of health behaviors and form motivation for change.During the preparation period,the social worker uses the power of role models to motivate the client to consolidate their willingness to change behaviors and make commitments for change.During the action period,the social worker assists the client in implementing behavioral changes by linking resources,strengthening management,and stimulating control.During the maintenance period,the social worker uses telephone and internet for follow-up to consolidate the results of behavioral changes in the client.This research combines qualitative and quantitative evaluation methods to evaluate the client before,during,and after intervention.Multidimensional evaluation can examine service effectiveness,the conclusions confirm that case work guided by the Transtheoretical Model of Change can effectively help the elderly with stroke sequelae form health awareness and develop health behaviors.
