| With the process of population aging,China has established a long-term care system that covers the "institutional,community,and home" elderly care model to meet the different needs of elderly people of all ages,and focuses on ensuring elderly care services for disabled and semi disabled elderly people.Due to the dual functions of elderly services and family support,community day care centers for the elderly have become one of the important factors in the selection of elderly people and their families,making the importance of community day care centers for the elderly in the long-term care service system of the community increasingly prominent.The needs of elderly people in community elderly day care centers exhibit diverse,complex,and long-term characteristics,and a single social work service method is difficult to respond to the practical needs of the elderly.Case management is applicable to elderly people and their caregivers who face complex situations and problems and have long-term daily care and medical rehabilitation nursing service needs.Its application scenario is highly consistent with the actual needs of elderly people in community elderly day care centers,laying the foundation for the application of case management in elderly day care centers.This paper takes the W case of Shenzhen D Community Day Care Center for the Elderly as the research object.Guided by the hierarchy of needs theory,community care theory,and Ecological systems theory,the case management method is used to provide comprehensive services for W case.The purpose is to solve the life care problems of service objects in the short term,alleviate the lack of communication between families,improve communication ability,and promote resource interaction in the community.Research has found that case management methods can effectively mobilize various resources in the environment to solve the difficulties of service recipients.It is concluded that case management intervention in community elderly day care center services can help individuals increase their abilities,promote family integration,improve center efficiency,and jointly build community friendliness.It is also summarized that case managers use the five senses method to establish professional relationships with elderly people in the process of case management services,Diagnose and evaluate the service targets of case management through self questioning and scale evaluation,use measurable and achievable strategies to determine service goals and develop service plans in stages,organize and organize resources,coordinate and mobilize comprehensive intervention interventions,and conduct supervision and evaluation throughout the entire process.Experience in the entire service process of result evaluation and outcome evaluation after case closure,Provide theoretical and practical references for social workers engaged in community elderly day care services.Finally,this article summarizes the application of case management in the D community elderly day care center in Shenzhen,and puts forward corresponding suggestions. |
