Objective: Recent, people have found that insulin-like factor 1 (IGF1) system is of more important role in human's pathophysiological progresses. The IGF1 system in cardiovascular system is more relative to atherosclerosis and coronary heart disease (CHD). IGF1 may take part in the pathophysiological process of atherosclerosis and CHD. However, the exact mechanism of pathophysiological process of atherosclerosis and CHD is not clear. Atherosclerosis has now been recognized an inflammatory disease by many scholars. The antigen, involved in atherosclerosis, may cause the inflammation, may injury blood vessels by means of immune reaction in body. Atherosclerosis and CHD may follow the inflammation and injury of blood vessels. Neopterin is a special maker of active monocytes/macrophages axis when stimulated by interferon- Y. It is a sensitive maker of active cell immune. The elevated concentration of neopterin was reported in patients with active immune diseases. The present studies investigated the concentrations of IGF1, neopterin, nitric oxide (NO), and endothelin (ET) in blood of CHD patients, to probepathophysiological process of the atherosclerosis and CHD, in order to explore the effects of these factors on the atherosclerosis and CHD.Method: 16 health donors and 40 CHD patients, confirmed by selective coronary arterial angiography, were investigated. The extent of coronary arterial narrowing was more or equal to 50% vessel cavity. The coronary artery is divided into three sub-arteries: left anterior descending artery, left circumflex artery, and right coronary artery. IGFl and ET concentrations were measured with radioimmunoassay. Neopterin concentration was measured with enzyme-liked immunosorbent assay (ELISA). NO concentration was measured with nitric acid restore enzyme method. The concentrations of IGFl, ET, neopterin, and NO in coronary vein sinus were measured with the same methods. The relationships among these parameters were analyzed using SPSS statistical software. The relationships between these parameters and the numberof diseased arteries were analyzed also using SPSS statistical software. Results: 1. The levels of elbow vein serum IGFl of CHD group were significantlylower than those in control group (respectively 27.23 ?15.48 ng/1 and 48.02 ?33.43 ug/1, PO.01). Though the much lower levels of IGFl, the more diseased coronary artery vessel numbers of coronary heart disease group, there was not a negative relationship between the serum IGFl levels and diseased coronary artery vessel numbers (r=-0.296, .P>0.05). The levels of IGFl in coronary vein sinus serum were significantly higher than those in elbow vein serum, of CHD group (respectively 44.17?5.58ug/l and 25.39?3.32ug/l, P<0.05). The much higher levels of IGFl, the more diseased coronary artery vessel numbers of CHD group, there was a positive relationship between the coronary vein sinus serum IGFl levels and diseased coronary artery vessels6(r=0.522, P<0.05). There was no difference in the levels of IGF 1 between acute coronary syndrome and stable angina.2. The elbow vein serum levels of neopterin of CHD group were significantly higher than those in control group (respectively 15.16?.44 nm/1 and 8.38+4.69 nm/1, P<0.01). There was a positive relationship between the serum neopterin levels and diseased coronary artery vessel numbers (r=0.378, PO.05). The levels of neopterin in coronary vein sinus serum and in elbow vein serum of CHD group were significantly higher than those in elbow vein serum of control group (respectively 21.63 + 16.04 nm/1 and 13.84?.18 nm/1 between 8.38?.69 nm/1, /><0.05 and /?0.01). The much higher levels of neopterin, the more diseased coronary artery vessels in CHD group, there was a positive relationship between the coronary vein sinus serum neopterin levels and diseased coronary artery vessels (r=0.539, PO.05).
