ObjectsBronchial asthma (asthma) is a common respiratory disease. It belongs to a kind of immuoreactions caused by many factors, and featured by reversible airway obstruction, airway hyperresponsiveness and airway inflammation. Patients with preoperative asthma history have obviously higher airway reactivity and inclined to induce bronchial spasm. A lot of factors during anesthesia and operation ( such as drug, transfusion, misinhalation, stimulation of operation and mechanics ) can lead to sudden stroke of asthma, which can even threaten patients lives.Intravenous anesthetics propofol and ketamine are often used in asthmatic patients during anesthesia induction and maintenance, based on their effects of airway dilation. The property of asthma is a chronic airway inflammation featured by airway hyperresponsiveness, and airway spasm in only one aspect of asthma. Previous studies often focus on the effects of these two drugs on airway dilation and little was known on the effects on lungs. People just presume that these two drugs can improve pathological changes of lungs, probably by relieving bronchial spasm, inhibiting microvascular leakage of lungs, eliminating oxygen radicals, decreasing the destruction of pulmonary surfactant, but few studies on them have ever been carried on.In view of the previous reasons, we modeled asthmatic rats, based on which we studied effects of propofol and ketamine on airway spasm and airway microvascular leakage by observing the change of airway pressure and airway dyecontent. We also studied effects of propofol and ketamine on pulmonary oxida-tive stress reaction by measuring pulmonary SOD and NOS activity, content of MDA and NO, iNOS and NT expression by the use of immunohistochemical method. Then we studied the effect of propofol and ketamine on pulmonary surfactant and pulmonary surfactant associated protein A (SP-A) by measuring the change of surface tension of bronchoalveolar lavage fluids ( BALF) , and content of SP - A by Western - blot. In addition, we recorded ATP - sensitive K+ currents (IKATP) ln single airway smooth muscle (ASM) cells of rats by the technique of patch clamp and further studied functional mechanism of propofol and ketamine from the point of ion channels.Our experiment consisted of four parts as following: 1) Effects of propofol and ketamine on antigen - induced airway bronchoconstriction and microvascular leakage in asthmatic rats; 2 ) Effects of propofol and ketamine on pulmonary oxi-dative stress in asthmatic rats; 3 ) Effects of propofol and ketamine on pulmonary surfactant and SP - A in asthmatic rats; 4) Effects of propofol and ketamine on IK ATP m smgle airway smooth muscle cells of asthmatic rats.Methods1. Materials; 90 SD rats were provided by experimental animal center of China Medical University. Main apparatus: RM -6200 physiological recorder, animal ventilator, spectrophotometer, two - stage vertical puller, dual beam memory oscilloscope, AD/DA converter, and pCLAMP 5.5. 1 software. Main reagent: ovalbumin ( OA ) , aluminum hydroxide, Bordetella pertussis, Evans blue (EB), iNOS antibody, Nitroglycerine (NT) antibody, collagenase, pro-nase, elastase, glibenclamide, Na2ATP, propofol, ketamine.2. Effects of propofol and ketamine on antigen - induced airway broncho-constriction and microvascular leakage in asthmatic rats: SD rats were sensitized 2 week before experiment with OA, aluminum hydroxide, and Bordetella pertussis. 30 rats were randomly allocated to one of 5 groups; c group received normal saline; P50 group received propofol 50 mg. kg-1, h-1; P100 group receivedpropofol 100 mg. kg -1. h-1; K50 group received ketamine 50 mg. kg-1, h-1; K100 group received ketamine 100 mg. kg-1, h-1. Evans blue dye was administered 30 min after anesthetic injection. OA was injected intravenously 5 min after dye injection. 30 min later, the experiment was finished and airway pressure, lung coefficient, wet/dry ratio, water content, and Evans blue content were measured.3.
