Experiment And Clinical Pilot Study Of Allergic Rhinitis Treated With Chinese Medicine On The Theory About "Treat The Disease Before Its Development"

Allergic Rhinitis (AR) is mediated by immunoglobutin E and is classified as a type I pathological reaction with typical symptoms of itchy,sneezing,sniveling and nasal blocking. It is a common disease in ENT department, and belongs to " Bi Qiu" of Traditional Chinese medicine (TCM). Now Chinese medicine has great prospect and potential in prevention and cure methods of Allergic Rhinitis. The research that about the effect of preventing and slowing down the symptom has important clinical and scientific significance.ObjectivesOn the theory about "Treat the Disease before its Development ", (1) Utilize animal experiment observe the animal's physical and chymic norm and symptom of rat with the Chinese medicine of warming kidney-yang in remissionstage, and discussing the possible mechanism of treating the AR with TCM in remissionstage through observing the change of cytokines. (2)Through the clinical case, treating the patient of deficiency of both lung and spleen with BIQIU Chinese medicine (warming lung and strengthening spleen) and application therapy, observe the change of the clinical symptom and the whole body sign in before and after taking medicine, confirmation the clinical effect of warming lung and strengthening spleen and application therapy to cure BIQIU with deficiency of both lung and spleen.Methods and Result1. Experiment study(1)MethodsUsing 32 small-male-rats in 8 weeks of C57/BL6 build the kidney-yang deficiency AR model with ovalbumin and adenine, divided into four groups. Treatment group is treated with warming kidney Chinese herbs. Control group is treated with NS. Observe the animal's physical and chymic norm and symptom; observing the nose symptoms of small rat in firstly 10 minutes after every dripping nose; test the Th, cytokines and Th2 cytokines by using ELISA; observe mastocyte and Eosnophils of the small-male-rat. The evaluation and statistics are done by using SPSS 17.0 for Windows software.And the datas are test by X2 test and T test.(2)ResultAnimal general condition:the kidney-yang deficiency group and the warming kidney-yang group appear lassitude, hair withered, hair dropping, chilly, curling, not moving, urorrhagia, and weight decline. After treating with the warming kidney-yang medicine (SHEN Ql WAN), the symptoms of the warming kidney-yang become better. Compared with the weight between two groups, there is statistics distinguish (p<0.05). The symptoms of scratching nose in the warming kidney-yang group is less than the kidney-yang deficiency group, there is statistics distinguish (p<0.05). Compared with the cytokines in blood serum, there is statistics distinguish between the kidney-yang deficiency group and AR group and between the warming kidney-yang group and AR group. There is statistics distinguish (p<0.05). Compared with histamine between in every two groups, there is no statistics distinguish (p>0.05). In nasal cavity perfusion liquid, there is no statistics distinguish between in every two groups. Except blank control group, the rest groups can find Eosnophils, but all groups are difficult to find mastocyte.2. Clinical study(1) Methods62 patients of BIQIU with deficiency of both lung and spleen who fitted diagnostic and fitted into remove rules were cured with warming lung and strengthening spleen (SHEN QI WEN) and application therapy (point application). All are treated for one month. Confirmation the clinical effect of warming lung and strengthening spleen and application therapy to cure BIQIU with deficiency of both lung and spleen. The evaluation and statistics are done by using SPSS 17.0 for Windows software.(2) Result HD and DF's positive rate are highest by allergern skin test. Among the patients,35 patients were found to be effective,26 patients were response, 1 patients were ineffective and the total effective rate amounts to 88.71%. Compared with the sympotoms before and after treatment, there is statistics distinguish (p<0.05). All patients's symptoms, for example, rhinocnesmus, sneeze, nasal discharge became better in varying degrees. Accompany symptoms, for example, chilly, spontaneous perspiration, lacking in strength, losing appetite, became better too. Symptoms integral reduce obviously.ConclusionThe study include two contains. One is Utilizing animal experiment observe the animal's physical and chymic norm and symptom of rat with the Chinese medicine of warming kidney-yang in remissionstage. The other is observing the clinical effect of the patient of BIQIU with deficiency of both lung and spleen who treated with Chinese herb. We can deduce (1) JIN GUI SHEN QI WAN can make IL4 down and make IFNr,IL2 up, can correct the unbalance cytokines network, make the internal T-cell differentiation reversion, promote the ratio of the cytokines that Th1/Th2 excreted recover balance; The Th2 cytokines (they are Il4) are higher it suggests that yang-deficiency-kidney has relationship with Th1/Th2 unbalance and warming-kidney can adjust the unbalance. And can induce Th1 cell differentiation, ripe and produce IL2, IFNr, restrain Th2 cell differentiation, ripe and IL4's produced, reduce IgE combining, adjust immune mechanism, then stop the happen of the allergy inflammation, in last reach therapeutical effect to AR.
