ObjectiveThis study was aimed to establish a realistic effect evaluation index system of AIDS health education and behavioral intervention through the qualitative and quantitative methods, in order to meet the needs of the assessment in the field of AIDS prevention and the promotion of the development of AIDS prevention and control, curbing the spread of AIDS from high-risk population groups to the general population. At the same time the index system was applied to assessment the effects of AIDS health education and behavioral intervention of different population in China's different regions.The interference pattern in different regions was summarized and analyzed. The modes of AIDS health education and ehavioral intervention effect of Sex female worker, drug users and adolecents were made to provide theoretical support and reference for the Chinese government in setting AIDS health education and behavioral intervention and long-term strategic planning.MethodsIn this study, literature searching, expert discusion, Delphi method to establish the index system, and Analytical Hierarchy Process (AHP) was used to calculate the weight coefficients of each indicator. The field survey method was utilized to evaluate the actual effect of the AIDS Health Education and Behavioral Intervention in different regions, and the effects were analyzed, the mode of the AIDS Health Education and Behavioral Intervention in different regions was summed up. Finally, researching methods such as insider interviews, panel discussions, the typical case analysis and literature review were utilized to put forward the mode of AIDS health education and behavioral intervention for Sex female worker, drug users and adolecents through viewing the domestic and international pattern of AIDS prevention and control.ResultsThe resultes of this study are as follows: (1) Three hierachical index systems were constructed, which were composed of four groups, namely prediposing, enabling, reinforcing facters and change of behaviors about AIDS-related health. The first class indicators of index system of effect evaluation of AIDS health education and behavioral intervention finally set up were the same for sexal female workers and drug users, including prediposing, enabling, reinforcing factors and change of behaviors; the same of their second class indicators included AIDS knowledge, health behavioral belief, health behavioral motivation, service capacity of health agencies, service utilization of health agences, AIDS policies and regulations, the internal enabling factors, external enabling factors, behavioural change in seeing doctor and the prevalence of STD and AIDS, sexual behavioral change and drug abuse behaviour change are different indicators in their index system; the numbers of the third class indicators in index system of sexual female workers were 40, in drug abuser were 37. The first class of indicator system of effect evaluation in adolecents included prediposing, enabling, reinforcing factors of behavior about AIDS-related health, and the second ones included AIDS knowledge, beliefs of health behavior, the attitude of health behavior, service capacity of educational agences, service utilization of educational agences, AIDS policies and regulations, the numbers of the third indicators are 21. (2) The index weights of index at three levels were calculated.â‘ female sexual services of a crowd The weights of the first class indicators of index system of female sex workers are 0.2624, 0.2472, 0.2289, 0.2596; the weight of the second class are 0.3622, 0.3277, 0.3101, 0.3346, 0.3332, 0.3322, 0.4989, 0.5011, 0.3344, 0.3312, 0.3343.â‘¡The weights of the first class indicators of drug users'index system are 0.2598, 0.2446, 0.2360, 0.2596; the weight of the second indicators are 0.3584, 0.3273, 0.3143, 0.3291, 0.3298, 0.3410, 0.4976, 0.5024, 0.3504, 0.3107 and 0.3389.â‘¢The weights of the first class indicators of adolecents'index system are 0.3360, 0.3400, 0.3240; The weights of the second class indicators of adolecents'index system are 0.3573, 0.3091, 0.3335, 0.3328, 0.3159, 0.3513, 0.4846, and 0.5154. (3) The established three sets of index system of effect evaluation of AIDS health education and behavioral intervention were applized to assessment the realistic situation in five cities of Hubei, Jiangsu and Fujian provinces.â‘ The actual score of effect assessment of AIDS health education and behavioral intervention are as follows: The scores of Wuhan, Xiangfan, Nanjing, Changzhou and Fuqing City are 86.08, 82.06, 74.77, 75.65 and 75.67, respectively, the actual comprehensive score of Wuhan City is the highest, the comprehensive of Nanjing city is slightly lower.â‘¡The actual score of effect assessment of AIDS health education and behavioral intervention are as follows: The scores of Wuhan, Xiangfan, Nanjing, Changzhou and Fuqing City are 85.13, 82.26, 84.81, 72.86 and 77.16, respectively, the actual comprehensive score of Wuhan City is the highest, the comprehensive ones of Changzhou City is slightly lower. (4) Based on the ecology of modern health promotion theory and the main theory of AIDS behavioral intervention, the AIDS health education and intervention model were proposed.ConclusionBased on the results of this study, the conclusions and recommendations could be put forward: (1) based on the ecology of marden health promotion and main behavioral intervention, the effect evaluation index system for AIDS education and behaviour intervention was constructed. According to the status of AIDS health education and behavioral intervention, the effect evaluation index system for AIDS education and behaviour intervention was established in multi-level and multi-dimention. (2) the effects of AIDS health education and behavioral intervention are evaluated in different rigions and population. Viwed from the four first-level index, the common problems existed: in behavioral prediposing and reinforcing index, the effects were good relatively; but behavioral enabling and chang index, the effects were bad relatively, there were such problems as poor multi-section cooperation, lack of funds, less numbers of peer education, generally low rate of VCT advise. It shows that AIDS prevention policy, lesdership development, multi-section cooperation mechanism and human resources shoud be emphasized in the future of AIDS heath education and behavioral intervention; (3) the demands for AIDS education and behaviour intervention in different population were analysed: because the demands in different population were different, it suggests that the different demands and behavioral characteristics shoud be stressed; (4) the ecology model of health promotion is the general pattern in AIDS education and behaviour intervention. It stresses environmemt chang, behavior and policy intervention to help people to make healthy choices in their daily lives. The spread of AIDS is closely related to people's behavior, which its nature is behavioral disease. The ultimate aim of AIDS education and behaviour intervention is to change people's risk behavior, prevent and control the spread of AIDS. According to the perspective of ecology of health promotion, multi-dimention (prediposing, enabling and reinforcing facters) and multi-level (individual, interperson and community levels) measures should be emphasized in AIDS education and behaviour intervention.Innovation(1) The theoretical frame of effect evaluation index system for AIDS health education and behavioral Intervention was explored. On the basis of ecology of modern health promotinon and main behavioral intervention theories, it's frame was established in personal, interpersonal and community levels, and the indicators were elected and flatered in prediposing, enabling and reinforcing facters.(2)The effect evaluation index system for AIDS health education and behavioral Intervention was Constructed. It was built up with scientific methods. The indicators of the index system covered acts of the main elements of behavioral intervention theory with more rationality, as well as, it provided the theoretical foundation for future research. (3)The models of AIDS health education and behavioral intervention were explored comprehensively. Through field survey, the models of different populations in different regions were reviewed and analysed. Based on ecology of modern health promotion and main behavioral intervention, the comprehensive models of behavioral intervention were exploered in multi-levels and multi-factors.
